五苓散对宫颈癌根治术后尿潴留的防治作用

The Preventive and Therapeutic Effect of Wuling Powder on Urinary Retention after Radical Cervical Cancer Surgery

目的探讨五苓散对宫颈癌根治术后尿潴留的防治作用。方法选取收治的85例宫颈癌患者,随机分为观察组(43例)和对照组(42例),患者均进行宫颈癌根治术,术后第5天对照组服用安慰剂,观察组服用五苓散,治疗3 d。观察尿潴留发生率、留置尿管总天数、第1次拔尿管后24 h内残余尿量、泌尿系感染率以及重置尿管率。结果观察组术后尿潴留的发生率为93.02%,对照组的为76.19%,观察组显著高于对照组(P<0.05)。观察组留置尿管总天数与对照组比较,差异不显著(P>0.05),2组患者拔尿管后24 h内膀胱残余尿量的比较,观察组显著低于对照组(P<0.01)。观察组泌尿系感染的发生率远低于对照组(P<0.01);而观察组的重置尿管率与对照组相比,差异不显著(P>0.05)。结论五苓散对宫颈癌根治术后尿潴留具有防治作用,降低了宫颈癌根治术后尿潴留的发生率,第1次拔尿管后膀胱残余尿量也显著降低,还降低了泌尿系感染的风险,值得在临床上应用。

Objective To explore the preventive and therapeutic effect of Wuling powder on urinary retention after radical operation of cervical cancer. Methods 85 patients with cervical cancer were randomly divided into observation group(n=43) and control group(n=42). Patients in each group underwent radical operation for cervical cancer. On the 5 day after operation, the control group took placebo, while the observation group took Wuling powder for 3 days. The incidence of urinary retention, total days of indwelling catheter, residual urine volume within 24 hours after first extubation, infection rate of urinary system and rate of replacing catheter were observed. Results The incidence of urinary retention after operation was 93.02% in the observation group and 76.19% in the control group. The incidence of urinary retention in the observation group was significantly higher than that in the control group(P<0.05). There was no significant difference in the total days of indwelling catheter between the observation group and the control group(P>0.05). The residual urine volume of bladder within 24 hours after the removal of catheter in the observation group was significantly lower than that in the control group(P<0.01). The incidence of urinary tract infection in the observation group was much lower than that in the control group(P<0.01), but there was no significant difference in the replacement rate of urinary catheter between the observation group and the control group(P>0.05). Conclusion Wuling Powder has preventive and therapeutic effect on urinary retention after radical operation of cervical cancer, reduces the incidence of urinary retention after radical operation of cervical cancer, and significantly reduces the residual urine volume of bladder after the first extubation of urinary catheter, and also reduces the risk of urinary tract infection. It is worthy of clinical application.
