丙泊酚复合瑞芬太尼在小儿麻醉维持喉罩通气中的应用效果

Application effect of Propofol combined with Remifentanil in pediatric anesthesia to maintain laryngeal mask ventilation

目的探讨丙泊酚复合瑞芬太尼在小儿麻醉维持喉罩通气中的应用效果。方法选取2018年10月~2019年3月我院收治的59例手术患儿作为研究对象,根据随机数字表法分为观察组(n=29)和对照组(n=30)。观察组患儿给予丙泊酚复合瑞芬太尼进行麻醉,对照组患儿给予右美托咪定复合瑞芬太尼进行麻醉。比较两组患儿手术过程中的心率(HR)、血氧饱和度(SpO2),手术完成后患儿唤醒情况以及不良反应发生情况。结果观察组患儿麻醉诱导后、喉罩置入、手术完成以及喉罩取出时的HR均低于对照组,SpO2高于对照组,差异有统计学意义(P<0.05);观察组患儿的Ramsay镇静评分低于对照组,差异有统计学意义(P<0.05);观察组患儿的不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论在小儿手术中,使用丙泊酚复合瑞芬太尼的整体效果优于右美托咪定复合瑞芬太尼,安全性较高,值得在临床推广应用。

Objective To investigate the application effect of Propofol combined with Remifentanil in pediatric anesthesia to maintain laryngeal mask ventilation.Methods A total of 59 children with surgery treated in our hospital from October 2018 to March 2019 were selected as the subjects,and divided into observation group(n=29)and control group(n=30)by random number table method.The observation group received anesthesia with Propofol combined with Remifentanil,and the control group received anesthesia with Dexmedetomidine combined with Remifentanil.The heart rate(HR),blood oxygen saturation(SpO2)during the operation,wake-up of the child after the operation and adverse reactions were compared between the two groups.Results The HR in the observation group after anesthesia induction,laryngeal mask insertion,operation completion and laryngeal mask removal were lower than those in the control group,and the SpO2 were higher than those in the control group,the differences were statistically significant(P<0.05).The Ramsay sedation score of children in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion In pediatric surgery with laryngeal mask ventilation,the overall effect of using Propofol combined with Remifentanil is better than that of Dexmedetomidine combined with Remifentanil,the safety is higher,and it is worthy of clinical application.
