摘要 目的探讨载药微球经动脉化疗栓塞(DEB-TACE)联合I125放射性粒子治疗肝细胞癌合并门脉癌栓的临床疗效。方法将30例肝细胞癌合并门脉癌栓患者根据治疗方法不同分为对照组(n=15)和研究组(n=15)。对照组采用DEB-TACE治疗,研究组采用DEB-TACE联合I125放射性粒子治疗。比较两组的临床疗效。结果研究组的客观反应率为66.67%,高于对照组的26.67%(P<0.05)。研究组的1年生存率为53.33%,高于对照组的13.33%(P<0.05)。研究组的OS、TTP长于对照组(P<0.05)。两组的腹腔感染、腹痛、肝肾功能损伤、出血发生率比较差异无统计学意义(P>0.05)。结论DEB-TACE联合I125放射性粒子治疗肝细胞癌合并门脉癌栓的近期疗效理想,且能够延长OS、TTP,不良反应少。 Objective To explore the clinical efficacy of transarterial chemoembolization with drug-eluting beads(DEB-TACE)combined with I125 radioactive particles in the treatment of hepatocellular carcinoma complicated with portal vein thrombus.Methods 30 patients with hepatocellular carcinoma complicated with portal vein thrombus were divided into control group(n=15)and study group(n=15)according to different treatment methods.The control group was treated with DEB-TACE,while the study group was treated with DEB-TACE combined with I125 radioactive particles.The clinical efficacy of two groups was compared.Results The objective response rate of study group was 66.67%,higher than 26.67%of control group(P<0.05).The 1-year survival rate of study group was 53.33%,higher than 13.33%of control group(P<0.05).The OS and TTP of study group were longer than those of control group(P<0.05).No statistical difference was found in the incidences of abdominal infection,abdominal pain,liver and kidney function damage and hemorrhage between two groups(P>0.05).Conclusions DEB-TACE combined with I125 radioactive particles in the treatment of hepatocellular carcinoma complicated with portal vein thrombus has ideal short-term efficacy,which can prolong the OS and TTP with few adverse reactions.
