EAAT2基因多态性对带状疱疹后神经痛病人疼痛敏感性的影响

Effects of EAAT2 gene polymorphism on pain sensitivity in patients with postherpetic neuralgia

目的:评价兴奋性氨基酸转运体2(excitatory amino acid transporter 2,EAAT2)基因多态性对带状疱疹后神经痛病人疼痛敏感性的影响。方法:采用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)的方法检测128例带状疱疹后神经痛病人和116例健康志愿者的EAAT2-rs4354668位点基因分型,分析带状疱疹后痛组病人EAAT2-rs4354668位点基因多态性与疼痛程度及阿片类药物用量的关系,对可能影响爆发痛发生的相关因素进行单因素分析及多因素Logistic回归分析。结果:①与健康对照组比较,带状疱疹后神经痛组病人rs4354668位点基因型频率和等位基因频率差异无统计学意义(P>0.05);②带状疱疹后神经痛组中,rs4354668位点中度疼痛和重度疼痛亚组各基因型和等位基因频率比较,差异均无统计学意义(P>0.05);③中、重度疼痛亚组中带状疱疹后神经痛病人不同基因型间阿片类药物用量比较差异无统计学意义(P>0.05);④带状疱疹后神经痛组中,rs4354668位点基因多态性与爆发痛发生之间无统计学意义(P>0.05)。结论:尚不能认为EAAT2基因多态性与带状疱疹后神经痛病人的疼痛敏感性有关。

Objective:To evaluate the effect of EAAT2 gene polymorphism on pain sensitivity in patients with postherpetic neuralgia.Methods:Polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)was used to detect the genotypes of EAAT2-rs4354668 in 128 patients with postherpetic neuralgia and 116 healthy volunteers.The relationship between EAAT2-rs4354668 polymorphism and pain intensity as well as that between EAAT2-rs4354668 polymorphism and opioid dosage in patients with postherpetic neuralgia were analyzed.The factors that may affect breakthrough pain in patients with postherpetic neuralgia were analyzed by univariate analysis and logistic regression model.Results:①Compared with the control group,there was no significant difference in genotype frequency or allele frequency at rs4354668 in patients with postherpetic neuralgia(P>0.05);②In the postherpetic neuralgia group,there was no significant difference in the genotypes and allele frequencies between the moderate pain subgroup and severe pain subgroup at rs4354668(P>0.05);③In the postherpetic neuralgia group,there was no significant difference in the consumption of opioids in different genotypes at rs4354668 in the moderate pain subgroup and severe pain subgroup(P>0.05).④There was no significant difference between rs4354668 polymorphism and breakthrough pain in postherpetic neuralgia group(P>0.05).Conclusion:It is not sure that the EAAT2 gene polymorphism is associated with pain sensitivity in patients with postherpetic neuralgia.
