B超引导下肝癌微波消融治疗原发性肝癌的疗效及不良反应

Curative Effect and Adverse Reactions of B-ultrasound Guided Microwave Ablation in the Treatment of Primary Liver Cancer

目的探究原发性肝癌治疗使用B超引导下肝癌微波消融治疗的疗效和对患者的毒副作用。方法回顾性分析78例原发性肝癌患者治疗资料,依据治疗方式不同分为对照组(n=37)和研究组(n=41),分别接受肝动脉栓塞化疗和B超引导下微波消融治疗。比较2组患者治疗后近期疗效,远期疗效,治疗前后肝功能,血清肝癌标志物以及毒副作用发生率。结果研究组近期疗效和远期疗效显著优于对照组(P<0.05)。2组患者治疗后总胆红素(TBil)、谷草转氨酶(AST)和谷丙转氨酶(ALT)等肝功能指标和肿瘤特异性生长因子(TSGF)、高尔基体糖蛋白73(GP73)、甲胎蛋白(AFP)、α-L-岩藻糖苷酶(AFU)等血清肝癌标志物水平较治疗前均明显变化(P<0.05),治疗后研究组患者上述肝功能指标和肝癌标志物变化水平显著低于对照组(P<0.05)。研究组患者毒副作用发生率(4.88%)低于对照组(27.03%),且差异具有统计学意义(P<0.05)。结论B超引导下肝癌微波消融治疗原发性肝癌近期和远期疗效较好,对肝功能影响较小,可显著下调肝癌标志物水平,毒副作用较小。

Objective To investigate the curative effect and side effects of B-ultrasound guided microwave ablation in the treatment of primary liver cancer.Methods The treatment data of 78 patients with primary liver cancer were retrospectively analyzed.The patients were divided into the control group(n=37)and the study group(n=41)according to different treatment methods.The 2 groups were treated with hepatic arterial chemoembolization and B-ultrasound guided microwave ablation,respectively.The short-term and long-term curative effects after treatment,liver function and serum liver cancer markers before and after treatment,and the incidence of toxic and side effects were compared between the 2 groups.Results The short-term and long-term curative effects of the study group were significantly better than those of the control group(P<0.05).Levels of liver function indicators such as total bilirubin(TBil),aspartate aminotransferase(AST)and alanine aminotransferase(ALT)and levels of serum liver cancer markers such as tumor-specific growth factor(TSGF),Golgi glycoprotein 73(GP73),alpha fetoprotein(AFP)andα-L-fucosidase(AFU)in both groups changed significantly after treatment(P<0.05).After treatment,changes of the above liver function indicators and liver cancer markers in the study group were significantly smaller than those of the control group.The incidence of toxic side effects in the study group was lower than that of the control group(4.88%vs 27.03%)(P<0.05).Conclusion The short-term and long-term curative effects of B-ultrasound guided microwave ablation of liver cancer are good for the treatment of primary liver cancer,which has few influences on liver function,and can significantly reduce the level of liver cancer markers.And the toxic side effects are small.
