胃镜联合腹部彩超在小儿腹型过敏性紫癜早期诊断中的应用研究

Application of Gastroscopy Combined with Abdominal Color Doppler Ultrasound in Early Diagnosis of Abdominal Allergic Purpura in Children

目的对胃镜联合腹部彩超在小儿腹型过敏性紫癜早期诊断中的应用效果进行调查。方法以郑州大学附属儿童医院110例腹型过敏性紫癜患儿而研究对象,所有患儿入院后均接受胃镜检查、腹部超声检查,对其单一检查阳性率进行统计同时对联合检查的阳性率进行统计。结果腹部超声检查阳性率为85.5%、胃镜检查阳性率为87.3%、联合检查阳性率为98.2%,P<0.05。胃镜漏诊有3例患儿无胃黏膜病变、有6例患儿仅出现回肠粘膜病变;5例误诊患儿均误诊为急性胃肠炎;腹部彩超漏诊患儿有4例未出现肠道病变、有6例患儿仅累及胃黏膜病变;6例误诊患儿均被误诊为肠套叠;联合检查漏诊患儿均为出现胃肠道变化。结论胃镜联合腹部彩超在小儿腹型过敏性紫癜患儿中的诊断率更高。

Objective To investigate the effect of gastroscopy combined with abdominal color Doppler ultrasonography in the early diagnosis of abdominal allergic purpura in children.Methods 110 children with abdominal allergic purpura in our hospital were studied.All children received gastroscopy and abdominal ultrasonography after admission.The positive rate of single examination was counted and the positive rate of combined examination was also counted.Results The positive rates of abdominal ultrasonography,gastroscopy and combined examination were 85.5%,87.3%and 98.2%,respectively,P<0.05.There were 3 cases of gastroscopic missed diagnosis without gastric mucosal lesions,6 cases only with ileal mucosal lesions;5 cases of misdiagnosed children were misdiagnosed as acute gastroenteritis;4 cases of abdominal color Doppler ultrasound missed diagnosis did not appear intestinal lesions,6 cases only involved gastric mucosal lesions;6 cases of misdiagnosed children were misdiagnosed as intussusception;all the missed children were misdiagnosed as gastrointestinal changes.Conclusion The diagnostic rate of gastroscopy combined with abdominal color Doppler ultrasound in children with abdominal allergic purpura is higher.
