输尿管镜下HLL与PL治疗输尿管结石的优劣差异

Quality Difference between HLL and PL Under Ureteroscope in the Treatment of Ureteral Calculi

目的:观察对比输尿管镜下钬激光碎石术(HLL)与气压弹道碎石术(PL)治疗输尿管结石的疗效差异。方法:选取2017年4月~2018年10月期间于某院泌尿外科行输尿管镜下碎石治疗的120例患者为研究对象,以随机数表法分为HLL组和PL组,各60例。HLL组给予HLL治疗,PL组给予PL治疗,观察两组患者经治疗后碎石成功率、结石排净率、手术时间、住院费用、住院天数、肌酐、尿素氮水平。结果:HLL组治疗碎石成功率、结石排净率显著高于对PL组(P<0.05);手术时间、住院天数、术后肌酐、尿素氮水平显著低于PL组(P<0.05);HLL组住院费用与PL组无显著差异(P>0.05)。结论:HLL治疗输尿管结石有显著疗效,碎石效果优于PL,同时有缩短手术及住院时间、改善肾功能等优势。

Objective:To observe and compare the efficacy of holmium laser lithotripsy(HLL)and pneumatic lithotripsy(PL)under ureteroscope in the treatment of ureteral calculi.Methods:120 patients who underwent ureteroscopic lithotripsy from April 2017 to October 2018 in the urology department of a hospital were selected as the study subjects.They were randomly divided into HLL group and PL group,60 cases in each group.HLL group was treated with HLL,and PL group was treated with PL.The success rate of lithotripsy,stone clearance rate,operation time,hospital expenses,days of hospitalization and creatinine and urea nitrogen level of the two groups were observed.Results:The success rate of lithotripsy and stone clearance rate in HLL group were significantly higher than those in PL group(P<0.05).The operation time,hospitalization days,creatinine and urea nitrogen levels after operation were significantly lower than those in PL group(P<0.05).There was no significant difference in hospitalization cost between HLL group and PL group(P>0.05).Conclusion:HLL is effective in treating ureteral calculi.The lithotripsy effect is better than PL.It also has the advantages of shortening the operation and hospitalization time and improving renal function.
