河南省县级临床重点专科建设综合能力评价及相关因素分析

Comprehensive Evaluation and Relative Factors Analysis of Key Clinical County-Level Specialties Construction in Henan Province

目的了解河南省首批县级临床重点专科建设前后综合能力与一级指标发展情况,分析综合能力发展主要相关因素,有针对性地提出建议。方法建立评价指标体系,采用TOPSIS法对河南省县级临床重点专科2011-2016年综合能力与一级指标(专科基础、技术队伍、医疗服务能力、医疗质量、科研与教学)进行评价,通过配对t检验对比专科建设前(2011-2013年)与建设后(2014-2016年)综合能力,并对一级指标评分与综合能力评分进行相关性分析。结果河南省首批县级临床重点专科2011-2016年综合能力提升,建设效果显著;专科基础、医疗服务能力、医疗质量在建设后提高,技术队伍和科研教学能力出现波动;与专科综合能力评价相关性显著的一级指标为医疗服务能力、医疗质量、科研与教学。结论河南省县级临床重点专科项目的实施对于提升县级医院及专科服务能力具有重要作用,专科应更加注重人才队伍建设的质量,强化人才培养能力;持续提升专科综合能力主要相关因素--医疗服务能力与医疗质量,促进专科在各个维度共同发展与县级医院整体医疗卫生服务水平的提高。

Objective To understand the development of comprehensive ability and first-level indicators before and after the construction of the first batch of county-level clinical key specialties in Henan Province,analyze the main relative factors of comprehensive ability development,and make targeted recommendations.Methods An indicator evaluation system was established,TOPSIS method was used to evaluate comprehensive ability and first-level indexes(specialized subject basis,technical team,service capability,health care quality,research and teaching)from 2011 to 2016,the comprehensive ability before(2011-2013)and after(2014-2016)construction were compared through paired t test,and the correlation between first-level index and comprehensive ability was analyzed.Results The comprehensive capacity of the first batch of county-level clinical key specialties has been improved from 2011 to 2016,and the construction effect is identified significantly.Specialist foundation,service ability and medical quality have been enhanced in the construction stage,fluctuations in the technical team and the ability of research and teaching has appeared.The first-level indexes,which have a significant correlation with the comprehensive ability evaluation of the specialty,are service capacity,medical quality,scientific research and teaching.Conclusion The implementation of county-level key clinical specialty project in Henan Province plays an important role in enhancing the service capacity of county specialties and hospitals.The specialized subject should pay more attention to the quality of talent team construction to strengthen the ability to cultivate talent;to continuously facilitating the development of medical service capacity and health care quality,which are the main related factors of comprehensive capacity of specialty,to promote the progress of the specialty in all dimensions and the advance of medical and health services of overall forms in county-level hospitals.
