当归四逆汤联合激光光凝治疗糖尿病黄斑水肿疗效及对黄斑中心厚度的影响

Effect of Danggui Sini Decoction combined with laser photocoagulation on diabetic macular edema and its effect on macular center thickness

目的探讨当归四逆汤联合激光光凝治疗糖尿病黄斑水肿的疗效及对黄斑中心厚度的影响。方法选择2014年8月—2017年12月在河北以岭医院眼科就诊的糖尿病黄斑水肿患者119例,根据治疗方法的不同分为研究组60例与对照组59例。对照组给予激光光凝治疗,每间隔4 d 1次,连续治疗4次;研究组在激光光凝治疗基础上口服当归四逆汤,连服20 d。观察2组治疗前及治疗20 d后视力、黄斑中心厚度变化,统计2组治疗效果及治疗期间并发症发生情况。结果治疗20 d后,2组患者的最佳矫正视力均明显高于治疗前(P均<0.05),且研究组明显高于对照组(P<0.05);2组患者黄斑中心厚度均明显低于治疗前(P均<0.05),且研究组明显低于对照组(P<0.05)。治疗20 d后,研究组总有效率为98.3%(59/60),治疗期间并发症发生率为3.3%(2/60),对照组总有效率和治疗期间并发症发生率分别为83.1%(49/60)和18.6%(11/59),研究组治疗总有效率明显高于对照组(P<0.05),并发症发生率明显低于对照组(P<0.05)。结论当归四逆汤联合激光光凝治疗糖尿病黄斑水肿可明显提高患者最佳矫正视力,降低黄斑中心厚度,整体治疗效果好,且并发症少。

Objective It is to explore the effect of Danggui Sini Decoction combined with laser photocoagulation on diabetic macular edema and its effect on macular center thickness.Methods A total of 119 patients with diabetic macular edema who were treated in the ophthalmology department of Hebei Yiling Hospital from August 2014 to December 2017 were selected and divided into study group(n=60)and control group(n=59)according to their treatment methods.The control group was treated with laser photocoagulation,once every 4 days,and the treatment was continued for 4 times.The study group was treated with Danggui Sini Decoction orally for 20 days based on the treatment in the control group.The changes of visual acuity and macular center thickness were observed before treatment and after 20 days of treatment in the two groups,and the effects of treatment were evaluated and complications during the treatment were counted.Results After 20 days of treatment,the best corrected visual acuity of the two groups were significantly higher than that before treatment(P<0.05),and the acuity in the study group was significantly higher than that in the control group(P<0.05);the macular center thickness of the two groups was significantly lower than those before treatment(P<0.05),the thickness in the study group was significantly lower than that in the control group(P<0.05).After 20 days of treatment,the total effective rate in the study group was 98.3%(59/60),the complication rate during the treatment was 3.3%(2/60),and the total effective rate in the control group and the complication rate during the treatment were 83.1%(49/60)and 18.6%(11/59),the total effective rate of treatment in the study group was significantly higher and the incidence of complications was significantly lower than that of the control group(P<0.05).Conclusion Danggui Sini Decoction combined with laser photocoagulation can significantly improve the patients’optimal corrected vision,reduce the macular center thickness in the treatment of diabetic macular edema,the overall treatment effect is good,and the complications are few.
