基于循证和德尔菲法构建老年住院患者谵妄预防策略

Construction on evidence-based prevention strategies on delirium for elderly inpatients based on Delphi method

目的构建适合我国国情的老年住院患者谵妄预防策略。方法基于循证医学系统评价法,检索国内外数据库获取原始文献,采用Johns Hopkins证据等级与质量评价方法评定文献,初拟老年谵妄预防策略问卷,然后采用德尔菲法构建老年住院患者谵妄预防策略。结果37篇文献纳入研究,通过两轮专家咨询构建老年谵妄预防策略,包括4个一级指标、16个二级指标、72个三级指标。两轮专家咨询的积极系数分别为92.00%、95.65%,专家的权威系数分别为0.85、0.84,第2轮专家咨询后各条目重要性的变异系数为0.00%~19.99%,可操作性变异系数为0.00%~29.16%。结论基于循证和德尔菲法构建的老年住院患者谵妄预防策略具有科学性和实用性,可用于指导临床护理实践。

Objective To construct the evidence-based prevention strategies on delirium for elderly inpatients based on Delphi method.Method Search domestic and foreign database for original document by evidence-based system evaluation method.Evaluate the articles by Johns Hopkins evidence grade and quality evaluation method.Design questionnaire on delirium for elderly and construct prevention strategies on delirium for elderly inpatients based on Delphi method.Result 37 articles are included in the research.Two rounds of expert consultation constructs prevention strategies of delirium for elderly,including 4 first grade indexes,16 second grade indexes and 72 third grade indexes.Active coefficients of two rounds of expert consultation are 92.00%and 95.65%.Specialist authority coefficients are 0.85 and 0.84.Coefficient of variation on importance of each item is 0.00%~19.99%and coefficient of variation on maneuverability is 0.00%~29.16%after the second round of expert consultation.Conclusion Construction on evidence-based prevention strategies of delirium for elderly inpatients based on Delphi method is scientific and practical,which can guide clinical nursing practice.
