摘要 目的建立成人隐匿性自身免疫性糖尿病(LADA)多抗体联合检测的液体芯片技术,探讨该方法的实验室可行性及临床价值。方法建立液体芯片技术多抗体检测的初步模型,检测糖尿病患者血清中GADA、IAA、ICA、IA-2A、ZnT8A五中抗体含量,将其结果与酶联免疫吸附法(ELISA)作比较。结果该液体芯片模型实验室评价性能价良好,所测GADA、IAA、ICA、IA-2A、ZnT8A各值与ELISA法所测值的相关系数r分别为0.986,0.979,0.964,0.958,0.944;液体芯片法五个指标阳性率达到了85%,单指标GADA阳性率为63.8%,ELISA法五个指标阳性率为77.5%。结论使用液体芯片法联合筛查GADA、IAA、ICA、IA-2A、ZnT8A可提高LADA的阳性诊断率,在检测精密度方面与ELISA相当,检测范围更广、灵敏度更高,标本用量少于,所需时间短,是LADA筛查的有潜力的新手段。 Objective To establish a liquid chip technology combined detection of multiple antibodies in latent autoimmune diabetes(LADA) in adults, and to explore the feasibility and clinical value of the method. Methods Liquid chip technology of multi-antibody detection model is established and used for detection of five antibody levels including GADA, IAA, ICA, IA-2 A, and ZnT8 A in diabetes in human serum, the results were compared with the enzyme linked immunosorbent assay(ELISA). Results The performance price of the liquid chip model laboratory is well evaluated. The correlation coefficients of the measured values of GADA, IAA, ICA, IA-2 A, ZnT8 A between the two methods were 0.986, 0.979, 0.964, 0.958, 0.944, respectively. The positive rate of five indexes of liquid chip method reached 85%, the positive rate of single index GADA was 63.8%, and the positive rate of five indexes of ELISA method was 77.5%. Conclusion Using liquid chip technology to screen GADA, IAA, ICA, IA-2 A, ZnT8 A can improve the positive diagnosis rate of LADA. In terms of detection accuracy, it is equivalent to ELISA, which has wider detection range, higher sensitivity, less dosage and shorter time. It is a potential new way for LADA screening.
