右室间隔部与右室心尖部起搏对慢性房颤合并心力衰竭患者左心功能及hs-CRP、NT-proBNP的影响 被引量:13

摘要 目的探讨右心室心尖部(RVA)和右室间隔(RVOT)起搏治疗对慢性房颤合并心力衰竭患者超敏C反应蛋白(hs-CRP)、N末端B型利钠肽前体(NT-proBNP)及心功能的影响。方法80例慢性房颤合并心力衰竭患者根据不同治疗方法分为RVA组和RVOT组各40例,比较两组术前术后的血流动力学及心功能指标的变化。结果术后30 d、60 d及1年RVA组心电图监测QRS波宽时限均显著长于RVOT组(P<0.01)。术后RVOT组左心室舒张容积明显低于RVA组,但RVOT组左室射血分数、左心室短轴率及心脏起搏量明显升高(P<0.05);两组术后心功能不全者级别越高hs-CRP、NT-proBNP指标越高,随访1年两组差异均无统计学意义(P>0.05)。结论RVOT起搏能改善慢性房颤合并心力衰竭患者心功能及血流动力学参数,术后中长期治疗效果好,更适合做起搏点。
