PKP术后邻近椎体骨折的相关因素分析 被引量:18

摘要 目的探讨经皮椎体后凸成形术(PKP)治疗单节段骨质疏松性椎体压骨折术后出现邻近椎体骨折的影响因素。方法回顾性分析自2017-01—2018-01行PKP手术治疗的151例骨质疏松性椎体压缩骨折,末次随访时15例出现邻近椎体压缩骨折(骨折组),136例未出现骨折(未骨折组)。先采用单因素分析与PKP术后邻近椎体骨折发生的影响因素,再采用多因素Logistic回归分析PKP术后邻近椎体骨折发生的危险因素。结果 151例均顺利完成手术并获得12个月以上随访。15例术后出现邻近椎体骨折,再次进行PKP手术治疗,效果良好。骨折组与未骨折组在性别、年龄、骨水泥注入量、椎体前缘高度恢复比值方面比较差异无统计学意义(P>0.05),在骨密度T值、术中骨水泥渗漏、术后是否进行抗骨质疏松治疗方面差异有统计学意义(P <0.05)。多因素Logistic回归分析结果显示,骨密度T值较低、术中骨水泥渗漏、术后未进行骨质疏松治疗是PKP术后邻近椎体骨折的独立危险因素。结论术中骨水泥渗漏、术后患者骨密度T值较低、术后未规范抗骨质疏松治疗是PKP术后邻近椎体骨折的危险因素,术中注入骨水泥时要尽量避免骨水泥渗漏,术后要加强抗骨质疏松治疗,降低患者术后相邻椎体骨折的风险。
