高龄女性原发性高血压患者血尿酸水平及其危险因素分析 被引量:2

摘要 目的探讨高龄(≥80岁)女性原发性高血压患者的血尿酸水平及其危险因素。方法选取高龄(≥80岁)女性原发性高血压患者共172例,进行临床资料的收集、体格检查及实验室检查,根据女性高尿酸血症(HUA)的诊断标准(血尿酸>360μmol/L)分为实验组49例和对照组123例,比较两组一般临床资料及各项实验室指标,采用logistic回归分析该人群HUA的危险因素。结果172例高龄女性高血压患者中HUA患病率为28.49%。实验组中尿素氮、血肌酐、三酰甘油水平高于对照组,肾小球滤过率(eGFR)、高密度脂蛋白胆固醇(HDL-C)、血红蛋白水平低于对照组,差异均有统计学意义(t分别=4.49、4.79、2.86、-5.41、-2.48、-2.45,P均<0.05)。校正了年龄、高三酰甘油血症、低HDL-C血症、糖尿病相关变量后,logistic回归分析提示肾功能异常、血管紧张素受体拮抗剂(ARB)类药物为该类人群HUA的危险因素(OR分别=6.18、3.23,P均<0.05)。结论肾功能异常、ARB类药物使用为高龄女性原发性高血压患者HUA的危险因素,其中肾功能影响最大。 Objective To investigate the level of hyperuricemia(HUA)in old(≥80 years old)female with primary hypertensive and its risk factors.Methods A total of 172 old female patients with primary hypertensive were selected and divided into HUA group(49 cases)and normal control group(123 cases)according to the HUA criteria.The clinic data between two groups were compared.The risk factors of hyperuricemia were analyzed by logistic regression analysis.Results The morbidity of HUA was 28.49%in the 172 patients.Compared with the normal control group,the levels of urea nitrogen,blood creatinine and triglycerine in the HUA group were higher,and the levels of eGFR,HDL-C and HB were lower(t=4.49,4.79,2.86,-5.41,-2.48,-2.45,P<0.05).After adjusting the age,hypertriglyceridemia,low HDL-C and diabetes,the logistic regression analysis showed that renal dysfunction and the angiotensin receptor blocker(ARB)drug were risk factors for HUA(OR=6.18,3.23,P<0.05).Conclusion Renal dysfunction and the ARB drug are the risk factors for hyperuricemia,among which renal dysfunction has the greatest impact.
