下气道菌群结构的变化与婴幼儿喘息的研究 被引量:1

摘要 目的探讨反复喘息患儿下气道菌群结构、丰度与多样性,为临床合理诊断及治疗提供理论依据。方法44例反复喘息患儿分为有异质性的反复喘息患儿(A组)、无异质性反复喘息患儿(N组)及无异质性因气道软化或狭窄反复喘息患儿(S组),收集支气管肺泡灌洗液(BLAF),提取基因组DNA并对细菌16S rRNA基因V4区进行PCR扩增,进行高通量测序生物信息学分析,比较不同体质反复喘息患儿下气道菌群结构的差异。结果3组患儿的下气道菌群的丰度及多样性差异无统计学意义(P>0.05);在门水平上,3组占主导地位的主要包括变形菌门Proteobacteria、厚壁菌门Firmicutes、软壁菌门Tenericutes;A组患儿与其余两组相比较,下气道菌群结构差异存在统计学意义(P<0.05)。结论反复喘息患儿下气道的菌群结构存在差异,菌群结构的失调可能为反复喘息患儿更易发展成哮喘的原因之一。 Objective To investigate diversity,richness,structure of lower airway microbiota in patients with recurrent wheeze,providing informative guide for clinic diagnose and treatment.Methods 44 children with recurrent wheeze recuited were divided to three groups.Those who were atopy were belonged to A Group.Those who were not atopy were belonged to N Group.Those who were bronchial stenosis or malacia were belonged to S group.DNA was extracted from their BALF and amplified for 16s rRNA gene sequencing on Illumina Miseq plat⁃form.Bioinformatic analysis on the microbiome was performed.Result There is no significant difference about richness and diversity of lower airway microbial community in three groups(P>0.05);Proteobacteria,Firmicutes,Bacteroidetes constituted three common dominant phylum in the three groups;A Group is different from the other two groups in lower airways microbial structure(P<0.05).Conclusions Significant changes in the structure of lower airways microbial occur in recurrent wheez children with atopy,The dysregulation of bacterial structure may be one of the reasons why recurrent wheezing children are more likely to develop asthma.
