应激性高血糖及糖尿病的糖化血清蛋白检测150例分析

Detection of glycosylated serum protein in stress hyperglycemia and diabetes mellitus: an analysis of 150 cases

目的:研究探讨糖化血清蛋白在应激性高血糖与糖尿病高血糖鉴别诊断中的应用价值。方法:选择75例糖尿病患者、75例血糖异常增高的危急重症患者以及75例正常体检健康者,分别设置为糖尿病组、应激性高血糖组、对照组,采集三组受检者的空腹静脉血液,检测和比较三组的空腹血糖、糖化血清蛋白、糖化血红蛋白水平。以临床确诊结果为参照,计算和比较糖化血清蛋白、糖化血红蛋白对应激性高血糖的鉴别诊断灵敏度、特异度、准确率。结果:糖尿病组与应激性高血糖组的空腹血糖比较,差异无统计学意义(P>0.05),而糖尿病组的糖化血清蛋白、糖化血红蛋白水平均高于应激性高血糖(P<0.05)。以临床确诊结果为参照,糖化血清蛋白、糖化血红蛋白对应激性高血糖的鉴别诊断灵敏度、特异度、准确率比较,差异均无统计学意义(P>0.05)。结论:糖化血清蛋白与糖化血红蛋白可对糖尿病高血糖与应激性高血糖予以鉴别区分,检测糖化血清蛋白、糖化血红蛋白可作为应激性高血糖的鉴别诊断手段。

Objective To study the value of glycosylated serum protein in differential diagnosis of stress hyperglycemia and diabetic hyperglycemia.Method 75 Diabetic patients,75 critical and severe patients with abnormal hyperglycemia and 75 normal controls were set as diabetic group,stress hyperglycemia group and control group,respectively.Fasting venous blood was collected from three groups.Fasting blood glucose,glycosylated serum protein and glycosylated hemoglobin were measured and compared.The sensitivity,specificity and accuracy of glycosylated serum protein and glycosylated hemoglobin in the differential diagnosis of stress hyperglycemia were calculated and compared.Results There was no significant difference in fasting blood glucose between the diabetic group and the stress hyperglycemia group(P>0.05),but the levels of glycosylated serum protein and glycosylated hemoglobin in the diabetic group were higher than those in the stress hyperglycemia group(P<0.05).Compared with the results of clinical diagnosis,the sensitivity,specificity and accuracy of glycosylated serum protein and glycosylated hemoglobin in the differential diagnosis of stress hyperglycemia were not significantly different(P>0.05).Conclusion Glycosylated serum protein and glycosylated hemoglobin can be used to differentiate diabetic hyperglycemia from stress hyperglycemia.Detection of glycosylated serum protein and glycosylated hemoglobin can be used as differential diagnostic methods of stress hyperglycemia.
