基于文献计量分析的我国医养结合领域问题重要性研究

Research on the Importance of Medical and Nursing Combination Based on Bibliometric Analysis

为了明确医养结合领域问题的主次关系,确定各问题对于系统目标实现影响力的大小,在文献计量分析的基础上,依据“卫生系统宏观模型”和供需理论得到24类影响医养结合领域发展的问题。通过对医养结合领域内23位专家进行两轮专家问卷获得问题重要性数据。结果发现,医养结合领域24类问题中,重要性指数排序前三位分别是:医养结合政策碎片化、内容局限、功能单一,贯彻落实不到位(0.1575);医养结合所需医生、护理、护工等专业人员缺乏(0.1148);医养结合管理碎片化,归口管理部门权责不明晰,无法形成有效监管(0.1118)。故认为,“医养结合政策碎片化,贯彻落实不到位”“医养结合医护人员短缺”“医养结合管理部门权责不清晰”为目前医养结合领域最重要的问题。有关部门应着重加强对这三类问题的重视,优先解决此三类问题。

In order to clarify the primary and secondary relationship of the problems in the field of medical and nursing integration,and determine the influence of each problem on the realization of the system objectives,based on the bibliometric analysis,24 kinds of problems affecting the development of the field of medical and nursing integration are obtained according to the“macro model of health system”and the theory of supply and demand.Through two rounds of expert questionnaires for 23 experts in the field of medical and nursing combination,the importance data of the questions were obtained.The results showed that among the 24 problems in the field of medical and nursing integration,the top three ranking of importance index were as follows:fragmentation of medical and nursing integration policy,limited content,single function,inadequate implementation(0.1575);lack of doctors,nurses,carers and other professional personnel(0.1148);fragmentation of medical and nursing integration management,unclear rights and responsibilities of the centralized management department,and failure to form an effective management system effective supervision(0.1118).Therefore,it is considered that the most important problems in the field of medical and nursing combination are“fragmentation of medical and nursing combination policy,inadequate implementation”“shortage of medical and nursing combination staff”and“unclear rights and responsibilities of medical and nursing combination management department”.The government and relevant departments should pay more attention to these three types of problems and give priority to solving them.
