掌长肌腱折叠联合抽出钢丝法治疗陈旧性锤状指

Treatment of old mallet finger by folding palmaris longus tendon and pulling out wire technique

目的探讨应用掌长肌腱折叠联合抽出钢丝法治疗陈旧性锤状指的临床疗效。方法自2010年6月至2017年6月,我们对19例19指伴有伸肌腱及止点缺损的陈旧性锤状指患者,将掌长肌腱游离并折叠后应用抽出钢丝法修复伸肌腱并重建伸肌腱止点。结果术后所有患者伤口均I期愈合,随访时间为3~18个月,平均6个月。根据Patel等的锤状指疗效评价标准评定:优7例,良9例,中2例,差1例。结论应用掌长肌腱折叠联合抽出钢丝法治疗陈旧性锤状指,材料简单,手术操作简便,患者手指功能恢复良好,临床效果满意。

Objective To investigate the clinical efficacy of folding palmaris longus tendon and pulling out wire technique in the treatment of old mallet finger.Methods From June 2010 to June 2017,19 cases(19 fingers)of old mallet fingers with extensor tendon and insertion defects were treated.The palmaris longus tendon was dissociated and folded,and the extensor tendon was repaired and the extensor tendon insertion was reconstructed by pulling out wire technique.Results All the wounds achieved primary healing after operation.The follow-up time ranged from 3 to 18 months with an average of 6 months.According to the Patel evaluation system for mallet finger,the results were rated as excellent in 7 cases,good in 9 cases,fanin 2 cases and poor in 1 case.Conclusion The method of folding palmaris longus tendon and pulling out wire technique for the treatment of old mallet finger is simple in material,simple in operation,good recovery of finger function and satisfactory in clinical efficacy.
