静脉注射对乙酰氨基酚在玻璃体切割术后的应用

Application of intravenous acetaminophen after vitrectomy

目的:探讨静脉注射在乙酰氨基酚对玻璃体切割术后的应用。方法:选取91例行单眼玻璃体切割术患者为研究对象,根据用药不同分为三组,分析三组术后疼痛、恶心呕吐、围手术期焦虑以及睡眠情况。结果:麻醉诱导前、插管1 min后、拔管前及离开麻醉恢复室前,三组患者血压及心率差异均无统计学意义(P>0.05);在麻醉恢复室、术后2 h、术后4 h同对照组相比,A组、B组VRS评分均明显偏低(P<0.05);对照组PONV人数明显多于A组和B组(P<0.05),且术后2天SAS、AIS评分明显高于A组和B组(P<0.05)。结论:静脉注射对乙酰氨基酚作为超前镇痛药物可以有效控制玻璃体切割术后疼痛和术后恶心呕吐,还可以有效缓解患者术后焦虑、有助于改善患者睡眠。

Objective:To investigate the application of intravenous acetaminophen after vitrectomy.Methods:91 patients undergoing single vitrectomy were selected as the study objects,divided into three groups upon different medications,the postoperative pain,postoperative nausea and vomiting(PONV),perioperative anxiety and sleep conditions of the three groups were analyzed respectively.Results:There were no significant difference in the blood pressure and heart rate among the three groups before anesthesia induction,1 minute after intubation,before extubation as well as leaving the recovery room(P>0.05);compared to the control group,there were lower VRS scores in Group A and Group B in the anesthesia recovery room,2 hours,4 hours and 24 hours after surgery(P<0.05),and the number presenting PONV of the control group was more than that of Group A and Group B(P<0.05);two days after operation,there were higher scores of SAS and AIS in the control group compared to Group A and Group B(P<0.05).Conclusion:Intravenous acetaminophen,as the medication for preemptive analgesia or prevention of pain,may effectively control postoperative pain and PONV of patients undergoing vitrectomy,in addition,it also may be effective in remission of postoperative anxiety and improvement of sleep quality of patients.
