PFNA治疗老年股骨转子间骨折的临床疗效

Clinical efficacy of PFNA in treatment of elderly patients with intertrochanteric fractures

目的探讨股骨近端防旋髓内钉(PFNA)治疗老年股骨转子间骨折的临床疗效。方法收集我院老年股骨转子间骨折患者58例,随机分为两组,观察组28例和对照组30例。观察组使用PFNA治疗,对照组采用动力髋钉(DHS)治疗,对比分析两组患者的临床疗效、手术时间、切口长度、术中出血量、术后卧床时间、术后引流量。结果根据Harris髋关节功能评分,观察组临床疗效优于对照组,P<0.05。两组观察指标比较,观察组的手术时间、切口长度、术中出血、术后卧床时间、术后引流量均明显少于对照组,P<0.05。结论PFNA治疗老年股骨转子间骨折具有术中出血少、手术时间短、创伤小、固定牢靠、疗效可靠等优点,是一种较为理想的治疗方法。

Objective To evaluate clinical efficacy of proximal femoral nail anti-rotation(PFNA)in the treatment of elderly patients with intertrochanteric fractures.Methods 58 elderly patients with intertrochanteric fractures were collected and randomly divided into the observation group(28 cases)and the control group(30 cases).The observation group was treated with PFNA and the control group was treated with dynamic hip nail(DHS).The clinical efficacy,the operation time,length of surgical incision,bleeding volume,postoperative bed-rest time,postoperative drainage volume were compared between the two groups.Results According to Harris hip joint function scale,the clinical efficacy of the observation group were significantly better than that in the control group(P<0.05).The operation time,length of surgical incision,bleeding volume,postoperative bed-rest time,postoperative drainage volume of the observation group were less than those in the control group(P<0.05).Conclusion PFNA has the characteristics of less bleeding,short operation time,less trauma,stable fixtion and better efficacy,which is a better choice for the treatment of elderly patients with intertrochanteric fractures.
