健肢主动运动对脑梗死介入术后患者跌倒恐惧和运动功能的影响 被引量:14

Effect of normal limbs active exercise rehabilitation training in fear of falling and motor function in patients with cerebral infarction after intervention

摘要 目的探讨健肢主动运动对脑梗死介入术后患者跌倒恐惧和运动功能的影响。方法选择2016年5月至2018年4月皖北煤电集团总医院收治的介入术后的74例急性脑梗死患者作为研究对象,按照随机数字表法分为观察组37例和对照组37例。对照组给予常规康复训练,观察组实施健肢主动运动。干预3个月后采用简明国际跌倒效能感量表(FES-I)评价患者跌倒恐惧程度,Barthel指数评定量表评价患者日常生活活动能力,简化Fugl-Meyer运动功能评分法评价患者肢体运动功能。结果干预前观察组和对照组简明FES-I得分为(15.32±3.15)、(15.47±4.89)分,干预后分别为(10.21±2.67)、(12.28±4.05)分;干预前观察组和对照组简明FES-I得分比较差异无统计学意义(P>0.05),干预后观察组简明FES-I得分明显低于对照组,差异有统计学意义(t值为2.468,P<0.05)。干预前观察组和对照组Barthel指数得分、上肢运动功能得分、下肢运动功能得分分别为(33.15±7.08)、(22.88±4.42)、(15.31±3.38)分和(33.40±3.78)、(22.26±5.03)、(16.04±3.30)分,干预后上述指标得分分别为(47.96±8.45)、(36.52±9.97)、(30.47±5.07)分和(43.23±6.58)、(31.10±4.53)、(24.24±6.06)分;干预前观察组和对照组Barthel指数、运动功能得分比较差异无统计学意义(P>0.05),干预后观察组Barthel指数、运动功能得分明显高于对照组,差异有统计学意义(t值为2.562、2.878、4.553,均P<0.05)。结论健肢主动运动可以降低脑梗死介入术后患者跌倒恐惧程度,提高其运动功能和生活自理能力。 Objective To investigate the effectiveness of normal limbs active exercise rehabilitation training in fear of falling and motor function in patients with cerebral infarction after intervention.Methods A total of 74 cerebral infarction after intervention patients were enrolled in Wanbei Coal-Electricity Group General hospital from May 2016 to April 2018.Patients were randomly divided into the observation group 37 patients and the control group 37 patients according to the random number table method.The control group received routine rehabilitation training,normal limbs active exercise rehabilitation was carried out in the observation group.After 3 months of intervention,the fear of falling,activities of daily living,limbs motor function was assessed by short Falls Efficacy Scale International(FES-I),Barthel index,Fugl-Meyer motor function scoring,respectively.Results Before intervention,the short FES-I score was(15.32±3.15)and(15.47±4.89)in the observation group and in the control group respectively,after intervention,the score was(10.21±2.67)and(12.28±4.05),respectively.There was no significant difference in short FES-I scores between the two groups pre-intervention(P>0.05).However,the short FES-I scores were significantly decreased in the observation group compared to the control group at post-intervention(t value was 2.468,P<0.05).Before intervention,the scores of Barthel index,upper limb motor function,lower limb motor function were(33.15±7.08),(22.88±4.42),(15.31±3.38)in the observation group,and(33.40±3.78),(22.26±5.03),(16.04±3.30)in the control group,however,those index mentioned above were(47.96±8.45).There was no significant difference in Barthel index,motor function scores between two groups pre-intervention(P>0.05).However,those scores were significantly increased in the observation group compared to the control group at post-intervention(t value was 2.562,2.878,4.553,P<0.05).Conclusion Normal limbs active exercise rehabilitation training can alleviate the fear of falling and promote motor function and self-care ability of patients with cerebral infarction after intervention.
