用磁共振成像检查与彩色多普勒超声检查诊断胎盘植入的效果对比

The effect of magnetic resonance imaging and color doppler ultrasound in diagnosing placental implantation was compared

目的:对比用磁共振成像(MRI)检查与彩色多普勒超声检查诊断胎盘植入的临床效果。方法:将2018年1月至12月期间在济南市章丘区人民医院就诊的70例疑似胎盘植入的孕产妇作为研究对象。对这70例孕产妇均进行彩色多普勒超声检查和MRI检查,然后根据其进行产后胎盘组织病理检查的结果,比较使用彩色多普勒超声检查与MRI检查对其发生胎盘植入的情况进行诊断的结果。结果:经产后胎盘组织病理检查的结果证实,在这70例孕产妇中,有24例孕产妇存在胎盘植入的情况。与进行产后胎盘组织病理检查相比,进行彩色多普勒超声检查诊断胎盘植入的Kappa值为0.87。与进行产后胎盘组织病理检查相比,进行MRI检查诊断胎盘植入的Kappa值为0.75。用这两种检查方法诊断胎盘植入的准确率与进行产后胎盘组织病理检查诊断胎盘植入的结果均具有一致性。对胎盘植入的孕产妇进行彩色多普勒超声检查和MRI检查的特异性、敏感性、阴性预测值、阳性预测值、假阴性率、假阳性率和精确比相比,差异无统计学意义(P>0.05)。结论:用MRI检查与彩色多普勒超声检查诊断胎盘植入的准确率均较高。在诊断胎盘植入时,需根据孕产妇的实际情况选择合适的检测方法。

objective:to compare the clinical effect of magnetic resonance imaging(MRI)and color doppler ultrasound in the diagnosis of placenta implantation.Methods:70 pregnant women with suspected placenta implantation who were admitted to zhangqiu district people’s hospital from January to December 2018 were studied.Color doppler ultrasonography and MRI examination were performed on all 70 pregnant women.Then,according to the results of postpartum placental histopathology,color doppler ultrasonography and MRI examination were used to compare the diagnosis results of placental implantation.Results:the results of postpartum placental histopathology confirmed that 24 of the 70 pregnant women had placenta implantation.Compared with postpartum placental histopathology,color doppler ultrasonography was used to diagnose placental implantation with a Kappa value of 0.87.Compared with postpartum placental histopathology,the Kappa value of placental implantation diagnosed by MRI was 0.75.The accuracy of these two methods was consistent with the results of postpartum placental histopathology.The specificity,sensitivity,negative predictive value,positive predictive value,negative rate,positive rate and accurate ratio of color doppler ultrasound and MRI examination of pregnant women with placenta implantation showed no significant difference(P>.05).Conclusion:the accuracy of MRI and color doppler ultrasound in diagnosing placenta implantation is higher.When diagnosing placenta implantation,it is necessary to choose the appropriate test method according to the actual situation of pregnant women.
