非瓣膜病心房颤动与CYP2C9基因多态性的相关性研究

摘要 目的研究汉族非瓣膜病心房颤动与CYP2C9基因多态性的相关性。方法自2017年9月至2019年3月收集来自于山西省不同地区就诊于山西医科大学第二医院心内科的汉族患者,本研究共纳入256例患者,男性134例,女性122例,将入选对象分为心房颤动组143例,对照组113例,所有入选研究对象均无血缘关系;分别对入选对象进行CYP2C9基因型检测,分析基因型与等位基因在两组间有无差异。结果非瓣膜病心房颤动组CYP2C9**l/*3基因型分布频率高于对照组,其中非瓣膜病心房颤动组为CYP2C9*1/*3基因型的患者占19例,对照组为CYP2C9*l/*3基因型的患者仅6例,差异有统计学意义(P=0.035);非瓣膜病心房颤动组CYP2C9*3等位基因分布频率高于对照组,其中非瓣膜病心房颤动组CYP2C9*3等位基因19个,对照组CYP2C9*3等位基因6个,差异有统计学意义(P=0.040)o二元Logistic回归结果示,排除年龄、低密度脂蛋白、高血压等因素的影响后,携带有CYP2C9*l/*3基因型的研究对象患非瓣膜病心房颤动的风险增加(OR=2.637,95%C/0.989-7.026)o结论CYP2C9基因多态性是汉族非瓣膜病心房颤动发生的危险因素。 Objective To study the correlation between atrial fibrillation and CYP2C9 gene polymorphism in non-valvular diseases of Han nationality.Methods From September 2017 to March 2019,a total of 256 Han patients,134 males and 122 females,were enrolled in this study.The subjects were divided into the atrial fibrillation group(143 cases)and the control group(113 cases).All the selected subjects were unrelated by blood.The selected subjects were tested for CYP2C9 genotype respectively and the difference between genotype and allele was analyzed.Results The distribution frequency of CYP2C9*l/*3 genotype in the non-valvular atrial fibrillation group was higher than that in the control group.Among them,there were 19 patients with CYP2C9*1/*3 genotype in the non-valvular atrial fibrillation group and only 6 patients with CYP2C9*l/*3 genotype in the control group,with a statistically significant difference(P=0.035).The distribution frequency of CYP2C9*3 alleles in the non-valvular atrial fibrillation group was higher than that in the control group,with 19 CYP2C9*3 alleles in the non-valvular atrial fibrillation group and 6 CYP2C9*3 alleles in the control group;the difference was statistically significant(P=0.040).The results of binary Logistic regression showed that after excluding the influence of age,low density lipoprotein,hypertension and other factors,subjects carrying CYP2C9*l/*3 genotype had an increased risk of non-valvular atrial fibrillation(O/?=2.637,95%CI 0.989-7.026).Conclusion CYP2C9 gene polymorphism may be a risk factor for non-valvular atrial fibrillation in Han nationality.
