慢性肾脏病患者心脏超声影像学特征分析

Analysis of Echocardiographic Characteristics of Heart in Patients with Chronic Kidney Disease

【目的】分析慢性肾脏病(CKD)患者心脏超声特点与其左房结构、容积及功能的关系。【方法】选择2016年1月至2019年2月在本院诊治的50例CKD患者(观察组)和50例健康体检者(对照组),采用彩色多普勒超声诊断仪检查,分析心脏超声特点与其左房结构、容积及功能的关系。【结果】观察组患者主动脉瓣流速明显高于对照组,肺动脉瓣流速显著低于对照组,其差异均有统计学意义(P<0.05);观察组患者舒张末期左室后壁厚度(LVPWd)、左室舒张末期前后径(LVIDd)、左房前后径(LAD)及舒张末期间隔厚度(IVSd)水平均明显高于对照组,差异有统计学意义(P<0.05);观察组左房最大容积指数(LAVImax)、左房最小容积指数(LAVImin)、左房收缩前容积指数(LAVIp)、左房总排空指数(LAVItotal)、主动排空指数(LAVIlact)、总排空分数(LATEF)及二尖瓣口舒张期早期血流峰速/组织多普勒舒张期二尖瓣环运动峰值(E/e')水平明显高于对照组,其差异有统计学意义(P<0.05);主动脉瓣流速和肺动脉瓣流速均与LVPWd、LVIDd、LAD、IVSd、LAVImax、LAVImin、LAVIp、LAVItotal、LAVIlact及E/e'水平呈显著正相关(P<0.05)。【结论】CKD患者心脏主动脉瓣流速和肺动脉瓣流速与左室结构、容积及功能呈显著正相关。

【Objective】To analyze the relationship between echocardiographic characteristics and left atrial structure,volume and function in patients with chronic kidney disease(CKD).【Methods】From January 2016 to February 2019,50 CKD patients(observation group)and 50 healthy people(control group)were selected for diagnosis and treatment in our hospital.The relationship between the characteristics of echocardiography and the structure,volume and function of left atrium was analyzed by color Doppler ultrasound.【Results】The velocity of aortic valve in the observation group was significantly higher than that in the control group,and the velocity of pulmonary valve in the observation group was significantly lower than that in the control group(P<0.05).The LVPWd,LVIDd,LAD and IVSd levels in the observation group were significantly higher than those in the control group The left atrial maximum volume index(LAVImax),left atrial minimum volume index(LAVImin),left atrial pre systolic volume index(LAVIp),left atrial total emptying index(LAVItotal),active emptying index(LAVIlact),total emptying fraction(LATEF)and peak velocity of blood flow in early diastolic period/tissue Doppler diastolic period(E/e')level was significantly higher than that of the control group(P<0.05),and the aortic valve velocity and pulmonary valve velocity were positively correlated with LVPWd,LVIDd,LAD,IVSd,LAVImax,LAVImin,LAVIp,LAVItotal,LAVIlact and E/e'(P<0.05).【Conclusion】There was a significant positive correlation between the velocity of aortic valve and pulmonary valve and the structure,volume and function of left ventricle in CKD patients.
