通络益气汤对带状疱疹后遗神经痛患者疼痛、睡眠及细胞免疫功能的影响

Effect of Tongluo Yiqi Decoction on Pain, Sleep and Cellular Immune Function in Patients with Postherpetic Neuralgia

目的:观察通络益气汤对带状疱疹后遗神经痛患者疼痛、睡眠及细胞免疫功能的影响。方法:将我院2015年12月~2017年12月收治的带状疱疹遗神经痛患者120例,采用随机化设计分为观察组(n=60)和对照组(n=60),对照组采用西药治疗,观察组采用我院自拟通络益气汤治疗,均治疗4周后比较两组临床疗效,以及治疗前后中医证候积分、数字评分法(NRS)疼痛评分及匹兹堡睡眠质量指数(PSQI)评分,检测两组治疗前后T细胞亚群CD3^+、CD4^+、CD8^+、及CD4^+/CD8^+水平,并比较不良反应。结果:观察组治疗总有效率为93.33%,显著高于对照组78.33%(P<0.05);治疗后两组症候积分均显著下降,且观察组显著低于对照组(P<0.05);治疗后两组NRS及PSQI评分均较治疗前显著下降,且治疗后不同时间点观察组NRS及PSQI评分均显著低于对照组(P<0.05);治疗后两组CD3^+、CD4^+及CD4^+/CD8^+水平均较治疗前显著升高,CD8^+水平均较治疗前显著降低,且观察组明显优于对照组(P<0.05);观察组不良反应发生率显著低于对照组(P<0.05)。结论:通络益气汤对带状疱疹后遗神经痛能显著提高临床疗效,改善临床症状,明显缓解疼痛及改善睡眠质量,提高患者细胞免疫功能,且副反应少。

Objective:To investigate the effect of Tongluo Yiqi Decoction on pain,sleep and cellular immune function in patients with postherpetic neuralgia.Methods:120patients with herpes zoster neuralgia treated in our hospital from December 2015to December 2017were randomly divided into observation group(n=60)and controlled group(n=60).The controlled group was treated with western medicine,while the observation group was treated with Tongluo Yiqi Decoction,which was prepared by our hospital.The clinical effects of the two groups were compared after 4weeks of treatment,as well as the scores of TCM syndromes and numerical scores(NRS)before and after treatment.Pain score and Pittsburgh Sleep Quality Index(PSQI)score were used to detect the levels of CD3^+,CD4^+,CD8^+,and CD4^+/CD8+of T lymphocyte subsets in the two groups before and after treatment,and to compare the adverse reactions.Results:The total effective rate of the observation group was 93.33%,which was significantly higher than that of the controlled group(78.33%,P<0.05);after treatment,the symptoms scores of the two groups decreased significantly,and the observation group was significantly lower than that of the controlled group(P<0.05);after treatment,the scores of NRS and PSQI in the two groups were significantly lower than those before treatment,and at different time points after treatment,the scores of NRS and PSQI in the observation group were significantly lower than those in the controlled group(P<0.05).The levels of CD3^+,CD4^+,and CD4^+/CD8^+in the observation group were significantly higher than those before treatment,and the levels of CD8+in the observation group were significantly lower than those in the controlled group(P<0.05),and the incidence of adverse reactions in the observation group was significantly lower than that in the controlled group(P<0.05).Conclusion:Tongluo Yiqi Decoction can significantly improve the clinical efficacy,clinical symptoms,pain relief,sleep quality and cellular immune function of patients with postherpetic neuralgia,with few side effects.
