“脑、腹四针”配合肌电生物反馈疗法治疗痉挛型脑瘫患儿疗效观察及对下肢肌力的影响

Efficacy of Applying “ Brain and Abdomen Four Needles ” Therapy Combined with Electromyographic Biofeedback Therapy in the Treatment of Children with Spastic Cerebral Palsy and Its Influence on Muscle Strength of Lower Limbs

目的:探究"脑、腹四针"配合肌电生物反馈疗法治疗痉挛型脑瘫患儿疗效观察及对下肢肌力的影响。方法:选取2015年9月~2018年1月我院收治的90例痉挛型脑瘫患儿为对象,按入院编号奇偶性将其均分为对照组和观察组各45例,对照组给予肌电生物反馈疗法治疗,观察组给予"脑、腹四针"配合肌电生物反馈疗法治疗,比较两组治疗后的临床疗效、痉挛分级、下肢肌力及粗大运动GMFM评分。结果:治疗前两组患儿的痉挛分级和屈髋肌力评分、伸膝肌力评分、踝背曲肌力评分比较均无明显差异(P>0.05),治疗后观察组的痉挛分级比较均优于对照组,屈髋肌力评分、伸膝肌力评分、踝背曲肌力均显著高于对照组,差异有统计学意义(P<0.05);治疗前、治疗1个月后两组患儿的粗大运动GMFM评分均无明显差异(P>0.05),治疗2个月、3个月后观察组的粗大运动GMFM评分均显著高于对照组,观察组总有效率为89.89%,明显高于对照组的66.67%,差异有统计学意义(P<0.05)。结论"脑、腹四针"配合肌电生物反馈疗法治疗痉挛型脑瘫患儿的临床疗效更显著,能够有效的改善患儿的下肢肌力、痉挛程度和运动功能,值得临床进一步推广。

Objective:To explore the efficacy of applying"brain and abdomen four needlesM therapy combined with electromyographic biofeedback therapy in the treatment of children with spastic cerebral palsy and its inflence on the muscle strength of lower limbs.Methods:90cases of children with spastic cerebral palsy admitted to our hospital from September 2015to January 2018were selected for the study and were divided into controlled group and observation group with 45cases in each group according to the parity of admission number.The controlled group was given electromyographic biofeedback therapy and the observation group was given "brain and abdomen four needles"therapy combined with electromyographic biofeedback therapy.The clinical efficacy,spasm grading,muscle strength of lower limbs,and gross motor GMFM score were compared between the two groups after treatment.Results:There was no significant difference in spasm grading and hip flexion muscle strength score,knee extension muscle strength score and flexor dorsal muscle strength score between the two groups before treatment(P>0.05),and the spasm grading in the observation group was better than that in the controlled group after treatment,and the hip flexion muscle strength score,knee extension muscle strength score and flexor dorsal muscle strength score were all significantly higher than those in the controlled group(P<0.05).There was no significant difference in the gross motor GMFM score before treatment at after 1 month of treatment between the two groups(P>0.05).After 2months and 3months of treatment,the gross motor GMFM score in the observation group was significantly higher than that in the controlled group,and the total effective rate in the observation group was significantly higher than that in the controlled group(89.89%vs.66.67%)(P<O.05).Conclusion:u Brain and abdomen four needlesM therapy combined with electromyographic biofeedback therapy for children with spastic cerebral palsy has significant efficacy,and it can effectively improve the muscle strength of lower limbs,spasm degree and motor function,therefore it is worthy of further clinical promotion.
