右美托咪定对区域麻醉的轻度抑郁老年患者术后短期认知功能的影响 被引量:2

Effects of dexmedetomidine sedation on short-term cognitive function in elderly patients with mild depression after regional anesthesia

摘要 目的探讨右美托咪定对区域麻醉的轻度抑郁老年患者术后短期认知功能的影响。方法将2016年5月~2019年5月我院收治行单侧髋关节置换术的200例轻度抑郁老年患者随机分成对照组、研究组,每组100例。麻醉时对照组静脉注射咪达唑仑,研究组静脉注射右美托咪定。对比观察两组患者不同时间点的血清S100β蛋白、神经元特异性烯醇化酶(NSE)水平;采用简易精神状态量表(MMSE),评价患者的认知功能,另外,观察统计两组患者的不良反应。结果与对照组比较,T1、T2、T3时,研究组血清S100β蛋白、NSE水平降低,术后1、3天时,MMSE评分升高(P<0.05);研究组与对照组的不良反应发生率并无统计学差异(P>0.05)。结论右美托咪定对区域麻醉的轻度抑郁老年患者术后短期认知功能影响较小,安全性高。 Objective To investigate the effect of dexmedetomidine on short-term cognitive function in elderly patients with mild depression after regional anesthesia.Methods From February 2018 to July 2019,200 elderly patients with mild depression who underwent unilateral hip replacement in our hospital were randomly divided into control group and research group,with 100 cases in each group.The control group was given midazolam intravenously and the study group was given dexmedetomidine intravenously.The levels of serum S100 beta protein and neuron specific enolase(NSE)at different time points were compared between the two groups.The cognitive function of the patients was evaluated by the Mini Mental State Scale(MMSE).In addition,the adverse reactions of the two groups were observed and counted.Results At T1,T2 and T3,compared with the control group,the levels of serum S100 beta protein and NSE in the study group was decreased,and the MMSE score was increased at 1 and 3 days after operation(P<0.05).There was no significant difference in the incidence of adverse reactions between the study group and the control group(P>0.05).Conclusion Dexmedetomidine has a positive effect on the short-term cognitive function of elderly patients with mild depression after regional anesthesia,with high safety.
