摘要
目的探讨小儿甲状腺癌超声检查参数与患儿病情发生发展的相关性。方法选取2016年1月至2019年1月间陕西省延川县人民医院收治的79例甲状腺癌患儿为实验组,另选取同期接受体格检查的80例甲状腺腺瘤患儿为对照组,两组患儿均进行二维超声、彩色多普勒及超声造影检查,比较两组患儿超声参数差异。结果实验组患儿甲状腺周边无回声、内部低回声、边界不清、微钙化灶及淋巴结转移率均明显高于对照组患儿,差异均有统计学意义(均P<0.05)。实验组患儿2~3级血流分布及血流参数收缩期峰值流速(PSV)和阻力指数(RI)均明显高于对照组患儿,差异均有统计学意义(均P<0.05)。实验组患儿造影参数中的峰值强度(PI)、达峰时间(TTP)和平均通过时间(MTT)均低于对照组患儿,差异均有统计学意义(均P<0.05)。结论超声检查能够较好的分辨小儿甲状腺病变类型,对其病情发生发展具有较好的判断价值。
Objective To explore the correlation between the parameters in ultrasonography examination of pediatric thyroid cancer and the disease occurrence and development.Methods Seven-nine children with thyroid cancer who were treated at People’s Hospital of Yanchuan County from January 2016 to January 2019 were selected as an experimental group,and 80 children with thyroid adenoma who underwent physical examination during the same period of time were selected as a control group.Two dimensional ultrasound,color Doppler and contrast-enhanced ultrasonography were performed in both groups.The differences in ultrasound parameters were compared between the two groups.Results The rates of peripheral anechoic,internal hypoechoic,unclear boundary,microcalcification and lymph node metastasis were significantly higher in the experimental group than in the control group(all P<0.05).The grade 2 to 3 blood flow distribution,peak systolic velocity(PSV)and resistance index(RI)were significantly higher in the experimental group than in the control group(all P<0.05).The peak intensity(PI),peak time(TTP)and mean transit time(MTT)were lower in the experimental group than in the control group(all P<0.05).Conclusion Ultrasound can better distinguish the types of thyroid lesions in children,and it can effectively predicting the occurrence and development of the disease.
