摘要
目的探讨超声速度向量(VVI)技术对妊娠期糖尿病患者早期心肌功能损伤的诊断价值。方法前瞻性地收集余姚市人民医院2017年1月至2018年11月收治的射血分数正常的妊娠期糖尿病患者50例为观察组,同期随机抽取正常妊娠期孕妇50例为对照组。观察两组的左心室基底段、中间段和心尖段的收缩期平均峰值速度、收缩期平均峰值应变、收缩期平均峰值应变率差异。结果两组左室射血分数比较差异无统计学意义[(64.37±6.37)%vs.(65.92±7.12)%,t=1.147,P=0.254]。与对照组比较,观察组左心室基底段、中间段和心尖段的收缩期平均峰值速度均显著降低(t值分别为3.036、2.838、2.226,均P<0.05)。与对照组比较,观察组左心室基底段、中间段和心尖段的收缩期平均峰值应变、收缩期平均峰值应变率均显著增高(t值分别为2.555、2.835、2.121、2.452、2.172、2.439,均P<0.05)。结论VVI技术对妊娠期糖尿病患者心肌功能损伤具有较高的敏感性,较常规超声更有利于早期识别患者的心肌损伤。
Objective To investigate the diagnostic value of ultrasound velocity vector imaging(VVI) in early myocardial damage for patients with gestational diabetes mellitus.Methods From January 2017 to November 2018,a total of 50 gestational diabetes patients with normal ejection fraction admitted to Yuyao People’s Hospital were prospectively collected as an observation group,and 50 cases of normal pregnant women were randomly collected as a control group during the same period.The differences of mean peak systolic velocity,mean peak systolic strain and mean peak systolic strain rate in basal,middle and apical segments of left ventricular between the two groups were observed.Results There was no significant difference in left ventricular ejection fraction between the two groups [(64.37±6.37)% vs.(65.92±7.12)%,t=1.147,P=0.254].The mean peak systolic velocity of basal,middle and apical segments of left ventricular in the observation group was significantly lower than those in the control group(t values were 3.036,2.838 and 2.226,respectively,P<0.05).And the mean peak systolic strain and mean peak systolic strain rate of the basal,middle and apical segments of left ventricular in the observation group were significantly higher than those in the control group(t values were 2.555,2.835,2.121,2.452,2.172 and 2.439,respectively,P<0.05).Conclusion Ultrasound velocity vector imaging technique is more sensitive to myocardial damage in patients with gestational diabetes mellitus,which is more helpful for early identification of myocardial damage than conventional ultrasound.
