高压氧辅助化疗对中晚期非小细胞肺癌患者的临床疗效及生活质量评分的影响

Clinical effects of HBO-assisted chemotherapy on the patients with advanced non-small cell lung cancer and their life quality

目的探讨高压氧(HBO)辅助化疗治疗中晚期非小细胞肺癌(NSCLC)患者的临床疗效及对生活质量评分的影响。方法选取2015年7月至2017年7月蓬莱市人民医院肿瘤科收治的88例中晚期NSCLC患者,按照入院时患者奇偶数编号分成观察组与对照组,每组44例。对照组患者给予单纯化疗和对症治疗,观察组患者在对照组治疗的基础上加用HBO治疗,治疗3个周期后比较2组患者的近期疗效,2年后通过生存曲线分析远期疗效。治疗28 d后,观察2组患者急性、亚急性毒性评分及对治疗前后生活质量评分进行比较。结果治疗28 d后,2组患者生活质量各维度及总均分均显著上调,且观察组患者各维度及总质量均分明显高于对照组(均P<0.05);观察组的肝肾功能、周围神经急性、消化道及血液系统急性、亚急性毒性评分均明显低于对照组(P<0.05)。治疗3个周期后,观察组患者的客观有效率(ORR)(77.27%)明显高于对照组(52.27%)(P<0.05)。随访2年,观察组和对照组患者中位疾病进展时间分别是9.6个月和6.9个月,中位生存时间分别是12.6个月和9.6个月,观察组均高于对照组,但差异均无统计学意义(P>0.05)。结论HBO辅助化疗可明显提高中晚期NSCLC患者的临床疗效及生活质量评分,减轻急性、亚急性毒性反应。

Objective To investigate the clinical efficacy of HBO-assisted chemotherapy in the treatment of advanced non-small cell lung cancer(NSCLC)and the effects on the scores of life quality Methods Eighty-eight patients with advanced NSCLC admitted into the hospital from July 2015 to July 2017 were divided into the observation group and the control group in accordance with the odd and even number of patient admission,each consisting of 44 patients.The patients in the control group were treated with chemotherapy and expectant treatment,while the patients in the observation group received HBO in addition to the treatment given to the control group.After 3 courses of treatment,short-term effects were compared between the patients of the 2 groups.Two years after treatment,Kaplan-Meier analysis was performed to evaluate the long-term treatment effects through the access of one-year survival curve.Twenty-eight days after treatment,acute and sub-acute toxic scores as well as the scores of life quality before and after treatment were closely observed and compared between the patients of the 2 groups.Results Twenty-eight days after treatment,various dimensions of life quality and total scores all significantly increased in the patients of both groups,with those of the observation group being obviously higher than those of the control group(all P<0.05).For the observation group,the scores of heptorenal function,the acute toxic scores of peripheral nerves,the acute and sub-acute toxic scores of digestive tract and blood system were all markedly lower than those of the control group(P<0.05).After 3 courses of treatment,ORR scores of the observation group(77.27%)were significantly higher those of the control group(52.27%)(P<0.05).After 3 courses of treatment,two-years follow-up indicated that medium disease progress time was respectively 9.6 and 6.9 months,and medium survival time was respectively 12.6 and 9.6 months,with those of the observation group being longer than those of the control group,but without statistical significance(P>0.05).Conclusion HBO-assisted chemotherapy in the treatment of the patients with NSCLC could obviously improve clinical efficacy and scores of life quality,and alleviate acute and sub-acute toxic reactions as well.
