联合独立成分和网络同质性分析对帕金森病患者默认网络及警觉功能的研究 被引量:3

Research on default mode network and alerting function in patients with Parkinson′s disease by combined independent component and network homogeneity

摘要 目的应用静息态功能磁共振成像(rs-fMRI)和注意网络测试(ANT)研究帕金森病(PD)患者默认网络(DMN)的网络同质性(NH)及警觉功能变化。方法对40例PD病患者(研究组)和45例性别、年龄、受教育年限相匹配的健康对照(对照组)进行全脑rs-fMRI扫描及ANT测试。通过独立成分分析(ICA)和NH分析方法研究PD患者的默认网络(DMN),将PD患者显示异常脑区的NH值与病程、各种警觉反应时间(RT)进行相关分析。结果与对照组相比,研究组右侧内侧额上回、双侧角回、左侧背外侧额上回、左侧额中回的NH值显著减低(P<0.05),双侧小脑上脚、左侧海马旁回、右侧颞下回、左侧颞中回、左侧楔前叶的NH值显著增高(P<0.05);与对照组相比,研究组的警觉功能中,包括位相性RT、固有警觉RT、警觉RT在内的各项警觉功能指标均明显延长(P<0.05)。但未发现研究组NH异常脑区的NH值与病程、各种RT有相关关系(P>0.05)。结论PD患者存在默认网络同质性异常以及警觉功能异常。 Objective To explore changes in network homogeneity(NH)of default mode network(DMN)and alerting function in patients with Parkinson′s disease(PD)by resting-state functional magnetic resonance imaging(rs-fMRI)and awareness network test(ANT).Methods The rs-fMRI scan of whole brain and ANT test were performed in 40 patients with Parkinson′s disease(the study group)and 45 healthy controls(the control group)matched by the same gender,age and educational background.The DMN of the patients with PD was analyzed by methods of independent component analysis(ICA)and NH analysis.Correlation analyses were made between the NH values and disease course and various alerting reaction time(RT)in abnormal brain areas of the patients with PD.Results Compared with those of the control group,the NH values of the study group significantly decreased in the right medial frontal gyrus,bilateral angular gyrus,left dorsolateral superior frontal gyrus and left middle frontal gyrus.The NH values of the study group significantly increased in the bilateral superior cerebellar peduncles,left hippocampus gyrus,right inferior temporal gyrus,left middle temporal gyrus and left precuneus.Compared with those of the control group,the alerting functions of the study group,including phase RT,inherent vigilance RT and vigilance RT,all significantly prolonged(P<0.05).However,no significant correlation was found between the NH values of the abnormal brain areas and the disease duration and various RT values(P>0.05).Conclusion There are abnormal homogeneity in the DMN and abnormal alerting function in the patients with PD.
