2012年-2018年某三甲传染病医院麻疹住院病例住院费用及影响因素分析

Analysis of hospitalization expenses and influencing factors of measles inpatient cases in a class a infectious disease hospital from 2012 to 2018

目的了解某院麻疹住院病例的住院费用构成情况,分析住院费用的影响因素。方法采集2012年-2018年某三级甲等传染病医院的麻疹住院病例,采用单因素分析和多元线性回归方法分析住院费用的主要影响因素。结果不同年龄和医疗付费方式的麻疹住院费用差异有统计学意义,年龄P<0.001,医疗付费P=0.002;麻疹患者人均住院天数为5.0天,人均住院费用为3272.3元,其中主要是药费和检查费,构成比分别为67.3%、18.5%;多元线性回归分析显示住院天数、合并症和年龄的标准偏回归系数分别为0.550、0.228和0.206。结论住院天数、合并症和年龄是麻疹住院费用的主要影响因素,在合理用药及检查的基础上,缩短住院天数及避免合并症的发生可以有效降低麻疹患者的住院费用。对成年人群实施麻疹疫苗的查漏补种可以有效预防麻疹的发生或减轻麻疹的病情,以降低住院费用。

Objective To understand the composition of hospitalization expenses of measles patients in a hospital and analyze the influencing factors of hospitalization expenses.Methods Measles inpatient cases in a Three A and Tertiary Infectious Disease Hospital from 2012 to 2018 were collected.Univariate analysis and multiple linear regression were used to analyze the main influencing factors of hospitalization expenses.Results There were statistically significant differences in the hospitalization expenses of measles patients with different age and medical payment methods,P<0.001,P medical payment=0.002.The average length of stay for measles patients was 5.0 days,and the average cost of stay was 3,272.3 yuan.Multiple linear regression analysis showed that the standard partial regression coefficients of length of stay,comorbidity and age were 0.550,0.228 and 0.206,respectively.Conclusions Length of stay,complications and age were the main influencing factors of measles hospitalization expense.On the basis of rational medication and examination,shortening the length of stay and avoiding complications could effectively reduce the hospitalization expense of measles patients.It was effective to prevent the occurrence of measles or reduce the illness of measles to reduce the cost of hospitalization.
