Analysis of DRGs Data in a Third-class A-level Chinese Medicine Hospital in Beijing

目的分析某三级甲等中医院近4年来部分DRGs数据指标,为医院未来发展及制定DRGs付费后应对策略提供参考。方法从某地区住院医疗服务绩效评价平台调取某中医院2015年1月-2018年12月的住院病案首页资料,提取分析病例数、例均费用、费用消耗指数、平均住院日、时间消耗指数、权重等DRGs相关信息,对相关数据信息进行列表统计分析。结果近4年该院出院人数前10位诊断相关分组的费用消耗指数、时间消耗指数及权重整体水平较稳定。其中OD35时间消耗指数、费用消耗指数均处较低水平,两者的均值都在0.5左右,但整体权重不高,平均值为0.16;IB23的时间消耗指数、费用消耗指数、权重均处较高水平,时间消耗指数平均值1.99,费用消耗指数平均值1.33,权重平均值1.69;NE19的时间消耗指数、费用消耗指数近年有上升趋势,时间消耗指数平均增幅2.71%,费用消耗指数平均增幅5.60%。结论出院人数具有优势的DRGs组的住院服务效率与产出之间发展不平衡,个别DRGs组住院服务效率不高,个别DRGs组住院产出偏低,个别DRGs组住院服务效率有逐年下降趋势。

Objective It analyzed some DRGs data indicators of a third-class A-level Chinese medicine hospital in Beijing in recent four years,and provided reference for the future development of the hospital and the formulation of DRGs post-payment coping strategies.Methods The DRGs related information,such as the number of cases,average case cost,cost consumption index,average length of stay,time consumption index,weight and so on,was extracted from the first page database of inpatient medical records of a hospital from January 2015 to December 2018,and the related data information was analyzed by tabular statistics.Results In the past four years,the overall level of cost consumption index,time consumption index and weight of the top 10 discharged patients in this hospital is relatively stable.Among them,OD35’s time consumption index and cost consumption index are at a low level,both mean values are about 0.5.The overall weight of OD35 is not high,with a 4-year average of 0.16.The time consumption index,cost consumption index and weight of IB23 are all at a higher level.The four-year average of time consumption index is 1.99,the four-year average of cost consumption index is 1.33 and the four-year average of weight is 1.69.The time consumption index and cost consumption index of NE19 have increased in recent years,with an average increase of 2.71%in four years and 5.60%in four years.Conclusions The relationship between inpatient service efficiency and outcomes in the diagnosis-related groups with superior discharges was unbalanced.The efficiency of inpatient service in individual diagnosis-related groups was not high.Inpatient output of Individual diagnostic relevant groups was low.The efficiency of hospitalization services in individual diagnostic related groups had been decreasing year by year.
