内上象限乳腺癌保乳术中应用邻近皮瓣转移即刻修复乳房缺损的效果及对患者生活质量的影响

Effect of immediate repair of breast defect with adjacent flap transfer in breast conserving surgery of upper quadrant breast cancer and its influence on the life quality of patients

目的评价内上象限乳腺癌保乳术中应用邻近皮瓣转移即刻修复乳房缺损的效果及对患者生活质量的影响。方法选取2016年5月~2018年12月于我院行手术治疗的100例女性乳腺癌患者作为研究对象,采用随机数字表法将其分为对照组与观察组,每组各50例。对照组患者采用乳腺癌改良根治术进行治疗,观察组患者在内上象限乳腺癌保乳术中应用邻近皮瓣转移即刻修复乳房缺损治疗。比较两组患者的手术情况、美容效果、生活质量、治疗总满意度以及不良事件总发生率。结果两组患者的手术时间、术中出血量以及住院时间比较,差异均无统计学意义(P>0.05);观察组患者的美容优良率、生理职能评分、心理健康评分均高于对照组,差异有统计学意义(P<0.05);观察组患者的治疗总满意度高于对照组,差异有统计学意义(P<0.05);两组患者术后的不良事件总发生率比较,差异无统计学意义(P>0.05)。结论女性乳腺癌患者在内上象限乳腺癌保乳术中应用邻近皮瓣转移即刻修复乳房缺损,可以显著改善患者的心理健康和生理职能,维护乳房美观,提高患者的治疗满意度,同时对手术及术后不良事件无显著影响,值得在临床中进一步推广。

Objective To evaluate the effect of immediate repair of breast defects by adjacent flap transfer during breast conserving surgery and quality of life in patients with inner upper quadrant breast cancer.Methods A total of 100 cases of patients with inner upper quadrant breast cancer treated in our hospital were selected as the subjects,and divided into control group and observation group according to random number table method,with 50 cases in each group.The patients in the control group were treated with modified radical mastectomy.The patients in the observation group received immediate repair of breast defects by adjacent flap transfer during breast conserving surgery.The surgical status,cosmetic effect,quality of life,the total treatment satisfaction and total incidence of adverse events of the two groups were compared.Results There was no significant difference in operation time,intraoperative blood loss and hospitalization time between the two groups(P>0.05).The good rate of cosmetic effect,physiological function and mental health scores in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).The total treatment satisfaction in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse events between the two groups(P>0.05).Conclusion For female breast cancer patients,the immediate repair of breast defects by adjacent flap transfer during breast conserving surgery can significantly improve the mental health and physiological function,maintain the beauty of the breast,enhance the satisfaction of treatment.And it has no significant impact on the operation and postoperative adverse events,which is worthy of further promotion in clinical practice.
