通过眼轴测量监控角膜塑形镜在青少年近视防控中的作用 被引量:9

Monitoring the role of orthokeratology lens in the prevention and control of myopia in adolescents by axial measurement

摘要 目的通过眼轴测量,探讨角膜塑形镜在青少年近视防控中的作用。方法选取2017年5月~2018年5月于本院就诊的120例青少年近视患者作为研究对象,按照随机数字表法将其分为观察组与对照组,每组各60例。观察组佩戴角膜塑形镜并以眼轴测量监控,对照组佩戴常规框架眼镜。干预过程均为1年,每3个月对患者进行随访,根据屈光度的改变更换眼镜。比较两组患者干预前及干预1年后的屈光度、角膜曲率、眼轴长度、裸眼视力、眼压;通过记录每3个月随访的测量数据,对眼轴与屈光度、角膜曲率、裸眼视力、眼压的关系进行线性相关分析。结果干预1年后,观察组患者的屈光度低于对照组,裸眼视力高于对照组,观察组患者的眼轴长度短于对照组,差异有统计学意义(P<0.05)。线性相关分析提示,眼轴与屈光度成正相关(r=0.783,P=0.002);眼轴与角膜曲率成负相关(r=-0.494,P=0.041)。结论角膜塑形镜能有效控制眼轴增长,改善青少年近视患者裸眼视力,具有较好的临床疗效及应用价值。 Objective Through eye axis measurement,the role of orthokeratology lens in prevention and control of juvenile myopia was discussed.Methods A total of 120 cases of juvenile myopic patients who visited our hospital from May 2017 to May 2018 were selected as research objects and divided into observation group and control group according to random number table method,60 cases in each group.Patients in the observation group were wearing orthokeratology lens and monitored by ocular axis measurement,while patients in the control group were wearing conventional frame glasses.The intervention process was one year.Patients were followed up every 3 months and glasses were changed according to diopter changes.The diopter,corneal curvature,axial length,uncorrected visual acuity and intraocular pressure of the two groups of patients before and one year after intervention were compared.By recording the measured data of follow-up every 3 months,the linear correlation analysis was carried out on the relationship between ocular axis and diopter,corneal curvature,naked eye vision and intraocular pressure.Results One year after the intervention,the diopter of the patients in the observation group was lower than that in the control group,and the naked eye vision was higher than those in the control group,the axial length of the patients in the observation group was shorter than that in the control group,the differences were statistically significant(P<0.05).Linear correlation analysis showed that the ocular axis was positively correlated with diopter(r=0.783,P=0.002).The ocular axis was negatively correlated with corneal curvature(r=-0.494,P=0.041).Conclusion Orthokeratology lens can effectively control the growth of ocular axis and improve the naked eye vision of juvenile myopia patients.It has good clinical efficacy and application value.
