四川省某院眼科日间住院患者情况分析

Analysis of Day-time Inpatients in Ophthalmology Department in a Hospital of Sichuan

目的了解四川省某院眼科日间住院患者的病种收治和退出日间情况,为有效地分配医疗资源和调整日间病房管理提供依据。方法回顾性分析2015年2月-2018年12月某院眼科日间病房住院患者,共计27 294例,调取患者的性别、年龄等人口学信息,疾病诊断、手术名称、手术取消原因等诊疗信息,统计日间手术人数、疾病类别、患者爽约率反映日间手术实施情况,使用Excel整理数据及统计学处理。结果 4年间日间手术量排在前5位的病种依次为晶状体疾病(47%)、脉络膜和视网膜疾病(15%)、眼肌调节(10.1%)、眼睑疾病(7.1%)和屈光疾病(4.5%)。退出日间病房的原因中最多的是爽约未到医院(82%),爽约原因前5位为放弃、改期、感冒、血压高、血糖高。预约后退出日间的患者占总人数11.1%。结论日间手术患者量逐年增多,加强日间病房的管理更显重要,退出日间病房以患者原因居多,进一步调整日间病房的管理模式和资源分配,优化预约入院制度,加强宣教和提高医师技术能力是解决问题的关键。

Objective To investigate the admission and withdrawal of day-time inpatients in ophthalmology department in a hospital of sichuan, so as to provide basis for effective allocation of medical resources and adjustment of day-time ward management. Methods A retrospective analysis was performed on a total of 27,294 inpatients from February 2015 to December 2018. Demographic information such as gender and age of the patients, diagnosis of diseases, operation name, reasons for cancellation of operations were collected.The number of patients undergoing daytime surgery, the types of diseases and the failure rate of patients were collected to reflect the implementation of daytime surgery. Excel collated the data and processed the statistics. Results In the past four years, the top five types of daily surgery were lens diseases(47%), choroidal and retinal diseases(15%), ocular muscle regulation(10.1%), eyelid diseases(7.1%) and refractive diseases(4.5%). The most common reasons for withdrawing from daytime ward were not arriving at the hospital(82%). The top five reasons for withdrawing were abandonment, rescheduling, cold, hypertension and hyperglycemia. Conclusion The number of patients undergoing daytime surgery is increasing year by year, and it is more important to strengthen the management of the day ward. The reasons for withdrawing from the day ward are mainly patients. The key to solve the problem is to further adjust the management mode and resource allocation of the day ward, optimize the system of appointment and admission, and strengthen the promotion and education of patients and the improvement of doctors’ technical ability.
