椎间孔镜配合经皮椎弓根螺钉内固定术治疗老年腰椎结核的疗效观察

Therapeutic effect of intervertebral foramen with percutaneous pedicle screw Hxation on treating elderly lumbar tuberculosis

目的:探讨椎间孔镜配合经皮椎弓根螺钉内固定术治疗老年腰椎结核疾病的安全性及有效性。方法:回顾分析桂林医学院第二附属医院骨外科收治的采用该手术方法治疗的老年腰椎结核患者15例。评价患者术后住院时间、手术时间及术中出血量、术后下地站立活动时间,比较手术前后VAS评分、神经功能分级等情况。结果:患者术后住院时间为(3.2±1.6)d,手术时间(136.2±18.7)min,出血量为(18.3±3.1)ml,术后(1.4±0.6)d患者即可下床站立活动。全部患者术后疼痛及神经症状均有明显改善,术后随访,患者未出现术后术口感染、伤口不愈合、腰椎结核复发等症状。结论:椎间孔镜配合经皮椎弓根螺钉内固定术治疗老年腰椎结核疗效满意,并发症少,是一种安全有效的治疗方法。

Objective:To investigate the safety and efficacy of intervertebral foramen with percutaneous pedicle screw fixation in the treatment of elderly lumbar tuberculosis.Methods:A retrospective analysis of 15 elderly patients with lumbar tuberculosis treated in our hospital with this surgical method was performed.Then the hospital stay,operation time and intraoperative blood loss,postoperative standing activity time of patients were evaluated,and the VAS score and neurological function grading before and after surgery were compared as well.Results:The postoperative hospital stay was(3.2±1.6)days,operation time was(136.2±18.7)min,blood loss was(18.3±3.1)ml;and(1.4±0.6)days after operation,the patient could get out of bed and stand for activity.All patients had significant improvement in pain and neurological symptoms.There found no postoperative wound infection,lumbar tuberculosis recurrence and other complications within 1 year after follow-up.Conclusion:Intervertebral foramen with percutaneous pedicle screw fixation for the treatment of elderly lumbar tuberculosis can gain satisfactory results and less complications,which is a safe and effective treatment.
