摘要
目的探讨3D CUBE T 1WI脂肪抑制序列对颅内静脉系统栓塞的诊断价值。方法收集我院临床怀疑颅内静脉血栓的30例患者,进行颅脑常规MRI平扫和MRV检查,同时加做颅脑3D CUBE T 1WI脂肪抑制序列扫描。由两位具有十年以上工作经历的放射科副主任医师,对所得图像进行诊断分析。结果12例(40%)MRV及3D CUBE T 1WI脂肪抑制序列图像均未发现明显异常信号影,经随访后证实非颅内静脉血栓;17例(约57%)为MRV显示异常,其中下矢状窦显示异常者4例(约13%),横窦及乙状窦显示异常10例(约33%),上矢状窦显示异常者3例(10%),3D CUBE T 1WI脂肪抑制序列未发现异常信号影,经随访证实为颅内静脉先天性变异;1例(约3%)MRI平扫发现脑表面水肿,3D CUBE T 1WI脂肪抑制序列图像发现异常高信号影,最终诊断为脑皮层小静脉的栓塞。结论3D CUBE T 1WI脂肪抑制序列对颅内静脉系统的先天性变异及颅内静脉系统栓塞的诊断具有较高的临床应用价值。
Objective To investigate the diagnostic value of 3D CUBE T 1WI fs sequence for intracranial venous system diseases.Methods From August 2018 to January 2019,there were 30 patients with suspected intracranial venous thrombosis in our hospital.In addition to routine MRI scan and MRV examination of the brain,they were also scanned by 3D CUBE T 1WI fat suppression sequence of the brain.The obtained images were diagnosed and analyzed by two deputy chief radiologists with more than 10 years'working experience.Results No obvious abnormal signal shadow was found in MRV and 3D CUBE T 1WI sequence images of 12 patients(40%).Non-intracranial venous thrombosis was confirmed after follow-up.17 cases(about 57%)showed abnormal MRV signal,among which 4 cases(about 13%)showed abnormal lower sagittal sinus,10 cases(about 33%)showed abnormal transverse and sigmoid sinus,and 3 cases(10%)showed abnormal upper sagittal sinus.One case(about 3%)was diagnosed with cerebral surface edema by MRI plain scan,and abnormal high signal shadow was found by 3D CUBE T 1WI fat suppression sequence image,which was finally diagnosed as cerebral venular embolism.Conclusion 3D CUBE T 1WI fs sequence is of great clinical value for congenital variation of intracranial venous system and embolization of intracranial venous system.
