彩色多普勒超声三血管气管切面诊断胎儿心脏大血管异常的临床价值 被引量:15

摘要 目的探讨彩色多普勒超声三血管气管切面在诊断胎儿心脏大血管异常的应用价值。方法选取我院诊断为心脏大血管异常的畸形胎儿73例患者作为观察对象,采用GE Voluson E8彩色多普勒超声诊断仪做血管气管切面(the three vessels and trachea view,3VT)扫描,探查胎儿心脏大血管情况。结果胎儿心脏大血管切面超声检查结果中,胎儿超声三血管异常检出率为97.26%(71例),其中管径异常检出35例(47.95%),血管位置发育异常19例(26.03%),血管数量异常17例(23.29%),与病理检测结果无明显差异,不存在统计学意义(P>0.05);超声诊断35例血管管径异常中,主动脉异常17例,肺动脉异常18例,漏诊1例轻度肺动脉瓣狭窄异常;19例血管位置异常中有18例血管位置顺序异常,漏诊1例与右位主动脉弓位置异常;17例患者血管数量异常中,14例血管数量增加,3例永存动脉干数量减少。结论彩色多普勒超声三血管气管切面诊断胎儿心脏大血管异常,可以得到更清晰准确的三血管气管结构,以更便捷有效。 Objective To explore the value of GE Voluson E8 color Doppler ultrasound in the diagnosis of fetal cardiac macrovascular abnormalities.Methods From August 2016 to December 2018,73 abnormal fetuses diagnosed as abnormal cardiac macrovessels were selected as the subjects of the study,the three vessels and trachea view,3VT(the three vessels and trachea view,3VT)was performed with GE Voluson E8 color Doppler ultrasound to explore the status of fetal cardiac macrovessels,including the position,quantity and diameter of cardiac macrovessels.Results Among the results of fetal cardiac macrovascular section ultrasonography,the abnormal detection rate of fetal three vessels was 97.26%(71 cases),including 35 cases(47.95%)with abnormal diameter,19 cases(26.03%)with abnormal vascular position and 17 cases(23.29%)with abnormal number of blood vessels.There was no significant difference between the results of pathological examination and that of pathological examination(P>0.05).Of the 35 cases with abnormal vessel diameter,17 cases of the aorta were abnormal,18 cases of pulmonary artery were abnormal,and 1 case of mild pulmonary valve stenosis was missed,and in 19 cases of abnormal vessel position,there were 18 cases of abnormal vessel position,1 case of missed diagnosis and the abnormal position of the right-position aortic arch.In 17 patients,the number of vessels increased,the number of vessels increased in 14 cases,and the number of the 3 cases of persistent artery decreased.Conclusion The diagnosis of fetal cardiac macrovascular abnormalities by GE Voluson E8 color Doppler ultrasound three-vessel trachea section can obtain clearly and more accurate three-vessel trachea structure,so as to improve the detection rate of fetal cardiac macrovascular abnormal development more conveniently and effectively,which is worth popularizing in related clinical examination.It is easy to master and can clearly display the anatomical structure of the three-vessel trachea,which is of great value for the examination of fetal cardiac macrovascular abnormalities.
