儿童感染性疾病血清铁血磷变化分析

摘要 目的检测感染性疾病儿童的电解质水平并分析原因及意义。方法选取2018年7月至2019年2月本院儿科收治的145例各类感染性疾病患儿作为观察组,选取同期在本院儿童保健科体检的112例健康儿童作为对照组,检测两组研究对象的电解质水平,进行比较分析。结果观察组血清铁水平(4.17±3.57)μmol/L,血磷水平(1.39±0.22)mmol/L;对照组血清铁水平(16.93±5.72)μmol/L,血磷水平(1.62±0.14)mmol/L。观察组血清铁水平明显低于对照组,差异具有统计学意义(t=20.690,P<0.05),观察组血磷低于对照组,差异具有统计学意义(t=9.742,P<0.05);两组血清铁、血磷检测结果均未超过参考范围的检测值;观察组中血清铁低于2μumol/L患儿白细胞水平(12.48±6.08)×10~9/L,血清铁高于2μmol/L儿童的白细胞水平(8.41±4.03)×10~9/L,比较差异具有统计学意义(t=-4.129,P<0.05)。两组钾、钠、氯、钙、镁的异常率比较差异无统计学意义。结论儿童在急慢性感染时,机体会存在磷、铁代谢紊乱,血清铁重新分布,血清铁明显下降,甚至低于检测下限2μmol/L,血清铁低于2μmol/L儿童白细胞水平高于血清铁大于2μmol/L的儿童,可为临床疾病的治疗提供一定参考依据。在临床上,在儿童急慢性感染期间,应综合其他相关检查结果分析血清铁在缺铁贫中的诊断价值,以进行临床指导,同时应关注并动态监测血磷避免患儿出现低磷状态导致不良后果。 Objective Detection of electrolyte level in children with infectious diseases and analysis of causes and significance.Methods 145 children with various infectious diseases admitted to our hospital from July 2018 to February 2019 were selected as the observation group. At the same time, 112 healthy children were selected as the control group,the electrolyte levels of the two groups were measured and compared. Results The serum iron level was(4.17±3.57)μmol/L, the serum phosphorus level was(1.39±0.22) mmol/L in the observation group, the serum iron level was(16.93±5.72) μmol/L and the serum phosphorus level was(1.62±0.14) mmol/L in the control group. The level of serum iron in the observation group was significantly lower than that in the control group(t=20.690, P<0.05). The blood phosphorus of the observation group was lower than that of the control group, the difference was statistically significant(t=9.742, P<0.05). The results of serum iron and phosphorus in the two groups were not beyond the reference range. The level of serum iron in the observation group was lower than that of children’s leucocytes(12.48±6.08)×10~9/L, the serum iron level was higher than that of 2 μmol/L children(8.41±4.03)×10~9/L, the difference was statistically significant(t=-4.129, P<0.05).There was no significant difference in the abnormal rate of K、Na、Cl、Ca、Mg between the two groups. Conclusion In children with acute and chronic infection, the metabolism of phosphorus and iron may be disordered, The concentration of serum iron decreased significantly, even lower than the detection limit of 2 μmol/L,The leukocyte level of children with serum iron lower than 2 μmol/L is higher than that of children with serum iron higher than 2 μmol/L, which can provide some reference for the treatment of clinical diseases. In clinic, during the period of acute and chronic infection in children, the diagnostic value of serum iron in iron deficiency should be analyzed based on the results of other relevant examinations to guide the clinical practice. At the same time, we should pay attention to and dynamically monitor the blood phosphorus to avoid the adverse consequences caused by the low phosphorus status of children.
