摘要
目的探讨伴有鼻腔解剖结构异常的阻塞性睡眠呼吸暂停综合征(OSAHS)患者个体化鼻腔扩容术后采用止鼾器的应用效果。方法选取2018年6月至2019年2月在本院就诊的50例OSAHS伴鼻腔解剖结构异常患者,根据患者个人意愿分为对照组(个体化鼻腔扩容术,26例)和观察组(个体化鼻腔扩容术联合止鼾器,24例),对比两组多导睡眠监测(PSG)结果、动脉血氧分压情况及匹茨堡睡眠质量指数量表(PSQI)、鼾声问卷(SOS)、Epworth嗜睡量表(ESS)评分。结果观察组治疗后呼吸暂停指数(AI)、呼吸睡眠紊乱指数(AHI)均低于对照组,动脉血氧分压较高,差异有统计学意义(P<0.05);观察组治疗后PSQI、SOS、ESS评分均低于对照组,差异有统计学意义(P<0.05)。结论伴鼻腔解剖结构异常的OSAHS患者在个体化鼻腔扩容术后采用止鼾器治疗可提高治疗效果,降低AHI、AI,提高动脉血氧分压,改善睡眠情况。
Objective To investigate the effect of snore arrester in patients with obstructive sleep apnea syndrome(OSAHS) with abnormal nasal anatomy after individualized nasal dilatation. Methods 50 patients with OSAHS accompanied by abnormal nasal anatomy from June 2018 to February 2019 were selected and divided into control group(26 cases of individualized nasal dilatation) and observation group(24 cases of individualized nasal dilatation combined with snoring arrester). The results of polysomnography(PSG), arterial oxygen partial pressure and Pittsburgh sleep quality index(PSQI), snoring questionnaire(SOS) and Epworth sleepiness scale(ESS) scores were compared between the two groups. Results After treatment, the Apnea Index(AI) and Respiratory Sleep Disorder Index(AHI) in the observation group were lower than those in the control group, and the arterial partial pressure of oxygen was higher(P<0.05). The PSQI, SOS and ESS scores in the observation group were lower than those in the control group, with statistical significant difference(P<0.05). Conclusion Snore arresting device can improve the therapeutic effect, reduce AHI and AI, increase arterial oxygen partial pressure and improve sleep in OSAHS patients with abnormal nasal anatomy after individualized nasal dilatation.
