甘肃地区1705例不育门诊男性患者精液质量及影响因素分析

Semen quality analysis of male infertility in Gansu province and their impact factors

目的分析甘肃地区不育门诊男性精液质量及影响因素,全面评估省内不同地区不育男性患者精液质量情况。方法收集兰州大学第一医院生殖医学专科医院门诊男性不育患者1705例并进行精液常规检测,根据年龄、民族、地区分组进行相关性分析。结果精液质量参数的中位数:体积3.00mL,pH7.20,密度67.52×10^6/m L,精子活动率62.14%,(a+b)级精子百分率35.41%。仅有21.06%的精液常规参数全部符合WHO标准。单因素分析提示,年龄在40岁以上的男性精液体积与其他年龄男性相比,精液体积显著下降(P<0.05)。多元线性回归结果显示,年龄是精液体积的主要影响因素,随着年龄的增长,精液体积有显著下降趋势(P<0.05);汉族男性精液pH相对回族及其他民族更高(P<0.05)。同时,我们发现陇南、白银两地少弱精子症患者的患病率最高,这可能与当地工业污染相关。结论甘肃省不育男性精液质量不容乐观。年龄、民族、地区差别及环境因素可能造成精液质量下降,具体机制仍需进一步研究。

Objective To investigate semen parameters of male infertility in Gansu province and their impact factors for better understand of semen quality of male infertility in different regions of Gansu province.Methods Total of 1705 male semen samples were collected from outpatient male infertility in our hospital and divided into different groups by age,nationality and region for correlation analysis.Results Medians of semen parameters were 3.00 mL for volume,7.20 for pH value,67.52×10^6 per m L for semen concentration,62.14%for total sperm vitality,35.41%for sperm progressive motility.Only 21.06%of semen parameters met WHO standard.Single factor analysis suggested that the semen volume of men over 40 years old was significantly lower than that of men of other ages(P<0.05).Multiple linear regression showed that age was the main influencing factor of semen volume,and with the increase of age,the semen volume exhibited a significant decrease trend(P<0.05).In addition,the pH of Han male semen was higher than that of Hui and other nationalities(P<0.05).Also,we found that Longnan and Baiyin had the highest prevalence rate of asthenospermia patients,which may be related to the serious pollution of local industry.Conclusion The semen quality of infertile men in Gansu province is not optimistic.Several factors may contribute to the decline of semen quality,such as age,nationality,regional differences and environmental factors,which needs to be further studied.
