摘要
目的:探讨阴虚型和湿热型原发性干燥综合征临床特点的差异。方法:分析277例阴虚型和湿热型原发性干燥综合征患者的临床资料,统计2组在性别、年龄、病史、系统累及、实验室指标、疾病活动等方面的差异。结果:2组患者在性别、烟酒史、家族史、抗核抗体阳性率、抗SSA抗体阳性率、抗SSB抗体阳性率、免疫球蛋白(IgG、IgM、IgA)、补体(C3、C4)、细胞因子[白细胞介素(IL)-1β、IL-2R、IL-6、IL-8、IL-10、肿瘤坏死因子-α(TNF-α)]、ESDDAI评分方面比较,差异无统计学意义(P>0.05)。但阴虚组患者平均年龄大于湿热组(P<0.05),总体病程长于湿热组(P<0.05),贫血的发生率高于湿热组(P<0.05),湿热组患者泌尿系统、神经系统、消化系统受累的发生率高于阴虚组(P<0.05),阴虚组患者的中医症状分级量化表评分高于湿热组(P<0.05)。结论:阴虚型和湿热型原发性干燥综合征在临床特点上存在一定差异。
Objective:To explore the differences of clinical characteristics between yin deficiency and damp heat primary Sjögren's syndromes.Methods:The clinical data of 277 patients with primary Sjögren's syndrome of yin deficiency type and damp heat type were analyzed.The differences of gender,age,history,system involvement,laboratory indexes and disease activity between the two groups were counted up.Results:There was no significant difference between the two groups in gender,tobacco and alcohol history,family history,anti-nuclear antibody positive rate,anti-SSA and anti-SSB antibody positive rate,immunoglobulin(IgG,IgM,IGA),complement(C3,C4),cytokines(IL-1β,IL-2R,IL-6,IL-8,IL-10,TNF-α),and ESDDAI scores(P>0.05).However,the average age of the patients in the yin deficiency group was older than that in the damp heat group(P<0.05),the overall course of disease in the yin deficiency group was longer than that in the damp heat group(P<0.05),the incidence of anemia in the yin deficiency group was higher than that in the damp heat group(P<0.05),the incidence of urinary system,nervous system and digestive system involvement in the damp heat group was higher than that in the yin deficiency group(P<0.05),and the TCM symptom grading scale score in the yin deficiency group was higher than that in the damp heat group(P<0.05).Conclusion:There are some differences in clinical characteristics between yin deficiency and damp heat primary Sjögren's syndromes.
