浙江省乙型病毒性肝炎相关疾病的经济负担

The economic burden of hepatitis B-related diseases in Zhejiang province

目的调查分析浙江省乙型病毒性肝炎(乙肝)相关疾病的经济负担。方法采用多阶段分层抽样方法,开展乙肝相关疾病医疗费用问卷调查;估算浙江省2017年乙肝相关疾病直接、间接和总经济负担。结果共调查乙肝相关疾病1149例,年均总经济负担为2.68万元,其中年均直接、间接经济负担分别为2.34万元(87.31%)、0.34万元(12.69%);各相关疾病的年均经济负担在0.20万元(乙肝携带者)-4.35万元(原发性肝癌)之间。估算浙江省2017年乙肝相关疾病为474.96万例-487.00万例,总经济负担为1273.88亿元-1306.15亿元;各相关疾病的总经济负担在0.12亿元(急性乙肝)-219.82亿元(慢性乙肝)之间。结论浙江省乙肝相关疾病经济负担较重,且随疾病严重程度增加而加重。需重点加强慢性乙肝患者的规范化管理。

Objective To estimate the economic burden of hepatitis B-related diseases in Zhejiang province.Methods We used a multi-stage stratified sampling method to recruit subjects for a questionnaire-based survey of medical costs of hepatitis B-related diseases;we estimated the direct,indirect,and total economic burden of hepatitis B-related diseases in Zhejiang in 2017.Results A total of 1149 cases of hepatitis B-related diseases were investigated;the average annual total economic burden was 2.68 ten-thousand yuan per case,with 2.34 ten-thousand(87.31%)and 0.34 ten-thousand(12.69%)yuan per case for average annual direct and indirect economic burdens,respectively.The average annual per case economic burden by disease ranged from 0.20 ten-thousand yuan(hepatitis B carriers)to 4.35 ten-thousand yuan(primary liver cancer).A total of 474.96-487.00 ten-thousand cases of hepatitis B-related diseases were estimated to have occurred in Zhejiang in 2017,yielding a total economic burden of 127.388-130.615 billion yuan.The total economic burden by disease ranged from 0.012 billion yuan(acute hepatitis B)to 21.982 billion yuan(chronic hepatitis B).Conclusion The economic burden of hepatitis B-related diseases was very large in Zhejiang,and increased with increasing severity of disease.We should strengthen management of chronic hepatitis B patients.
