基于数据挖掘的全国名中医范永升教授治疗结缔组织病相关间质性肺病用药规律探析

Analysis on the Medication Law of Professor FAN Yongsheng in the Treatment of Connective Tissue Disease-related Interstitial Lung Disease Based on Data Mining

[目的]基于中医传承辅助平台,探讨范永升教授治疗结缔组织病相关间质性肺病(connective tissue disease-related interstitial lung disease,CTDILD)的用药规律。[方法]采用数据挖掘方法,分析范永升教授治疗CTD-ILD的药物频次规律及组方规律等,提炼范永升教授治疗CTD-ILD的用药经验。[结果]范永升教授治疗合并间质性肺病的结缔组织病包括系统性红斑狼疮、干燥综合征、硬皮病、皮肌炎、混合性结缔组织病等,临证多以化湿、通络、清热、宣肺、益气阴等为基本治法。本研究总结分析了治疗CTD-ILD的201首方剂,共应用中药166味。其中,应用温药、寒药、甘味药和苦味药较多。使用频次在100以上的中药有5味,分别是茯苓、白术、桔梗、白芍、甘草。常用组合有白术和茯苓,白芍和茯苓,白术和白芍,白术、茯苓和白芍,黄芪、白术和茯苓等。两味中药组合支持度大于50%,置信度大于80%有3对,分别是桂枝与黄芪、桂枝与白芍、桂枝与茯苓。三味中药组合支持度大于50%,置信度大于80%且位于前3位的分别是白术、桂枝对茯苓;桂枝、茯苓对黄芪;桂枝、茯苓对白术。[结论]范永升教授治疗CTD-ILD临床经验丰富,常甘温与苦寒药物并用,同时注重调肺、顾护脾胃,兼以理气。

[Objective] To investigate the medication law of Professor FAN Yongsheng in the treatment of connective tissue disease-related interstitial lung disease(CTD-ILD) based on the platform of traditional Chinese medicine inheritance assistance. [Methods] Data mining method was used to analyze the frequency and prescription of Professor FAN Yongsheng in the treatment of CTD-ILD, and to refine Professor FAN Yongsheng’s experience. [Results]Professor FAN Yongsheng treats CTD-ILD including systemic lupus erythematosus, scleroderma, dermatomyositis, mixed connective tissue disease, etc.Professor FAN often makes removing dampness, clearing collaterals, clearing heat, promoting lung and benefiting Qi and Yin as basic treatment methods.This study summarized and analyzed 201 prescriptions of CTD-ILD, and applied a total of 166 flavors of traditional Chinese medicine. Among them, there were more applications of warm medicine, cold medicine, sweet medicine and bitter medicine. There were 5 kinds of traditional Chinese medicines used for more than 100 times, which were Poria cocos, Atractylodes macrocephala, Radix platycodonis, Radix paeoniae Alba and Glycyrrhiza uralensis Fisch respectively. Those commonly used combination were Atractylodes macrocephala and Poria cocos;Radix paeoniae Alba and Poria cocos;Atractylodes macrocephala and Radix paeoniae Alba;Atractylodes macrocephala, Poria cocos and Radix paeoniae Alba;Astragalus membranaceus, Atractylodes macrocephala and Poria cocos, etc. There were three pairs of combination of two traditional Chinese medicines with support degree more than 50% and confidence degree more than 80%, including Cinnamomum cassia Presl and Astragalus membranaceus, Cinnamomum cassia Presl and Radix paeoniae Alba, Cinnamomum cassia Presl and Poria cocos. The top three pairs of combination of three traditional Chinese medicines with support degree more than50% and confidence degree more than 80% were Atractylodes macrocephala and Cinnamomum cassia Presl to Poria cocos;Cinnamomum cassia Presl and Poria cocos to Astragalus membranaceus,Cinnamomum cassia Presl and Poria cocos to Atractylodes macrocephala. [Conclusion] Professor FAN Yongsheng has rich clinical experience in the treatment of CTD-ILD, often uses warm sweet medicine in combination with cold bitter medicine. At the same time, he focuses on regulating lung, protecting spleen and stomach, and regulating Qi.
