普罗帕酮与胺碘酮对阵发性心房颤动患者血清超敏C反应蛋白和肿瘤坏死因子-α及白细胞介素-6水平的影响 被引量:2

The Effects of Propafenone and Amiodarone on the Levels of Serum High Sensitive C-Reactive Protein, Tumor Necrosis Factor-α and Interleukin-6 in Patients with Paroxysmal Atrial Fibrillation

摘要 目的探讨普罗帕酮与胺碘酮对阵发性心房颤动患者血清超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)水平的影响。方法选取2018年1月至2019年2月保定市第二中心医院收治的108例阵发性心房颤动患者作为研究对象,根据随机数字表法分为对照组和观察组,各54例。对照组予以普罗帕酮治疗,观察组采用胺碘酮治疗,比较两组转复时间、心室率、收缩压、舒张压,及治疗前后hs-CRP、TNF-α、IL-6水平。结果治疗后,观察组转复时间长于对照组,心室率、收缩压、舒张压低于对照组,差异有统计学意义(P<0.05);治疗后,观察组hs-CRP水平(2.81±0.34)μg/L、IL-6(33.97±5.16)mg/L、TNF-α(21.05±2.54)μg/L均低于对照组(5.23±0.49)μg/L、(51.48±6.24)mg/L、(37.14±4.26)μg/L,差异有统计学意义(P<0.05)。结论胺碘酮与普罗帕酮治疗阵发性心房颤动具有良好疗效,胺碘酮降低血清炎症介质水平疗效更为显著,但普罗帕酮见效时间更短,临床可根据实际情况合理用药。 Objective To explore the effects of propafenone and amiodarone on the levels of serum high-sensitivity C-reactive protein(hs-CRP), tumor necrosis factor-α(TNF-α), and interleukin-6(IL-6) levels in patients with paroxysmal atrial fibrillation. Methods A total of 108 patients with paroxysmal atrial fibrillation admitted to the Second Central Hospital of Baoding from January 2018 to February 2019 were enrolled in the study. According to the random number table method, they were divided into control group and observation group, with 54 cases in each group. The control group was treated with propafenone. The observation group was treated with amiodarone injection. The conversion time, ventricular rate, systolic blood pressure, diastolic blood pressure, and hs-CRP, TNF-α, IL-6 levels were compared between the 2 groups before and after treatment. Results After treatment, the conversion time of the observation group was higher than that of the control group. The ventricular rate, systolic blood pressure and diastolic blood pressure were lower than the control group(P<0.05). The hs-CRP level of the observation group was(2.81±0.34) μg/L, IL-6(33.97±5.16) mg/L, TNF-α(21.05±2.54) μg/L, which were lower than the control group(5.23±0.49) μg/L,(51.48±6.24) mg/L,(37.14±4.26) μg/L(P<0.05). Conclusion Amiodarone and propafenone have good curative effects in the treatment of paroxysmal atrial fibrillation. Amiodarone is more effective in reducing the level of serum inflammatory mediators, but propafenone has shorter effective time, so it can be used reasonably according to the actual situation.
