中药当归补血汤加减治疗肿瘤化疗所致贫血对患者预后的影响

Effects of Danggui Buxue Decoction on the Prognosis of Patients with Chemotherapy-Caused Anemia

目的探讨和分析中药当归补血汤加减对肿瘤化疗所致贫血患者预后的影响。方法选取2017年6月-2018年6月我院收治的60例肿瘤相关性贫血患者为研究对象,采用随机数字表法分为对照组(n=30)和观察组(n=30)。对照组患者采用常规化疗方案,观察组患者在对照组的基础上联合中药当归补血汤加减治疗。比较两组患者的治疗效果,包括平均血红蛋白浓度(MCHC)、红细胞(RBC)、血红蛋白(Hb)、网织红细胞(Rtc)及总铁结合力(TIBC)等临床指标水平的变化,以及患者的免疫功能改善情况、用药后不良反应与治疗不同时段的预后效果。结果治疗后,观察组患者临床Hb、MCHC及RBC水平显著高于对照组(P<0.05),TIBC、Rtc水平显著低于对照组(P<0.05),贫血症状明显改善;观察组患者CD3^+、CD4^+水平及CD4^+/CD8^+比值显著高于对照组(P<0.05),CD8^+水平显著低于对照组(P<0.05);观察组患者贫血与中医症候的疗效明显优于对照组(P<0.05),且用药后第1周、第4周、第8周的QOL评分均显著高于对照组(P<0.05),但两组患者治疗后的不良反应发生率无明显差异(P>0.05)。结论采用中药当归补血汤加减治疗肿瘤相关性贫血的临床治疗效果理想,可缓解患者的临床症状,改善其免疫功能,提高血红蛋白水平,用药安全性较高,预后良好,可在临床推广使用。

Objective To explore and analyze the effect of modified Danggui Buxue decoction on the prognosis of anemic patients caused by tumor chemotherapy.Methods A total of 60 patients with cancer-related anemia who were admitted to our hospital between June 2017 and June 2018 were selected in this study.The patients were divided into two groups by the application of digital random table method:control group(n=30)and observation group(n=30).Patients in control group were treated with routine chemotherapy,while those in observation group got Chinese medicine Danggui Buxue decoction on the basis of routine chemotherapy.Compare the treatment effects between the two groups,including the changes of clinical indicators such as mean hemoglobin concentration(MCHC),red blood cell(RBC),hemoglobin(Hb),reticulocyte(Rtc)and total iron binding capacity(TIBC).The improvement of immune function,the incidence of adverse reactions after medication and the prognostic effects during different treating periods were also compared between the two groups.Results After treatment,the RBC,Hb and MCHC levels of the patients in the obsenvation group were higher than those of the control group(P<0.05).The levels of TIBC and Rtc were significantly lower than that of the control group(P<0.05).The anemia symptom of the obsenvation group was significantly improved.In the observation group,the levels of CD3^+,CD4^+,and the CD4^+/CD8^+were significantly higher than those in the control group(P<0.05),while the level of CD8^+was lower than that in the control group(P<0.05).Moreover,the efficacy of anemia and TCM syndromes in the observation group was significantly better than that in the control group(P<0.05).The QOL scores in the first week,the fourth week and the eighth week after treatment were significantly higher than those in the control group(P<0.05).However,there was no significant difference in the incidence of adverse reactions after treatment between the two groups(P>0.05).Conclusion The Chinese medicine Danggui Buxue decoction had good clinical effects on patients with tumor-related anemia.It could relieve the clinical symptoms,increase the hemoglobin levels and improve the immune function.It has high drug safety and good clinical prognosis.
