食管癌术后家庭肠内营养治疗效果分析

Effect Analysis of Home Enteral Nutrition for Patients With Esophageal Cancer after Surgery

目的比较食管癌术后家庭肠内营养组与非家庭肠内营养组的临床疗效,探索食管癌患者是否从家庭肠内营养中获益。方法回顾性收集2018年2月22日-12月31日在江苏省肿瘤医院行食管癌根治术的88例患者的临床资料,根据是否给予家庭肠内营养,分为家庭肠内营养组(观察组)和非家庭肠内营养(对照组),各44例。比较两组患者出院1个月后的体重、体重指数(BMI)、血红蛋白和血清白蛋白等营养指标、丙氨酰氨基转移酶(ALT)和门冬氨酸转移酶(AST)等肝功能指标、血清尿素氮和血清肌酐等肾功能指标的变化,以及营养耐受性。结果出院1个月后,观察组患者体重和BMI的下降程度显著低于对照组(P<0.05);两组患者血清营养指标、肝肾功能和营养耐受性比较,差异均无统计学意义(P>0.05)。结论在医保费用允许的前提下给予家庭肠内营养,有助于维持食管癌术后患者的体重和BMI,并改善其营养状况,使患者获益更多。

Objective To compare the clinical efficacy of home enteral nutrition support therapy and non-home enteral nutrition support therapy after esophageal cancer surgery,and to explore whether home enteral nutrition support therapy benefits patients with esophageal cancer.Methods A retrospective study was conducted on 88 patients who underwent radical resection of esophageal cancer in Jiangsu Cancer Hospital between February 22,2018 and December 31,2018.According to patients getting home enteral nutrition or not,88 patients were divided into home enteral nutrition group(n=44)and non-home enteral nutrition group(n=44).Compare the changes of body weight and BMI,liver and kidney function(glutamic pyruvic transaminase,serum urea nitrogen,etc.),serum nutrition indexes(albumin,hemoglobin level,etc.)and nutritional tolerance between the two groups at one month after discharge.Results Patients in the two groups had the weight and BMI decreased at one month after discharge.But the weight and BMI of the home enteral nutrition group decreased less than that of the non-home enteral nutrition group(P<0.05).There were no statistical differences in serum nutrition index,liver and kidney function and nutritional tolerance between the two groups(P>0.05).Conclusion Under the premise that medical insurance cost allows,home enteral nutrition can control the decline of body weight and BMI and improve the nutritional status of patients.
