腹腔镜术式急诊治疗胃穿孔患者的效果

Effects of laparoscopy in emergency treatment of gastric perforation

目的观察腹腔镜术式急诊治疗胃穿孔患者的效果。方法选取广饶县人民医院普外科收治的100例急诊胃穿孔患者作为研究对象,依据随机数字表法分为对照组(n=50)和观察组(n=50)。对照组进行开腹修补手术治疗,观察组进行腹腔镜术式治疗。比较两组治疗效果、手术时间、肛门排气时间、术中出血量、总住院时间、手术前后胃泌素水平及并发症发生率。结果手术后观察组胃泌素水平明显优于对照组,差异有统计学意义(P<0.05);观察组手术时间、肛门排气时间及总住院时间均明显短于对照组,术中出血量明显少于对照组,差异均有统计学意义(P<0.05);观察组并发症发生率为2.00%,明显低于对照组的20.00%,差异有统计学意义(P<0.05)。结论腹腔镜术式急诊治疗胃穿孔患者可减少术中出血量,缩短手术和恢复时间,降低并发症发生率,效果优于开腹修补术治疗效果。

Objective To observe effects of laparoscopy in emergency treatment of gastric perforation.Methods 100 patients with emergency gastric perforation admitted to the general surgery department of Guangrao County People's Hospital were selected as the research objects,and were divided into control group(n=50)and observation group(n=50)according to the random number table method.The control group underwent laparotomy,while the observation group underwent laparoscopy.Then,the treatment effects,operation time,anal exhaust time,intraoperative blood loss,total hospital stay,gastrin levels before and after the surgery,and complications were compared between the two groups.Results After the surgery,the gastrin level in the observation group was significantly better than that in the control group,and the difference was statistically significant(P<0.05).The operation time,anal exhaust time and total hospital stay in the observation group were significantly shorter than those in the control group;the intraoperative blood loss was significantly less than that in the control group;and the differences were statistically significant(P<0.05).The complication rate in the observation group was 2.00%,which was significantly lower than 20.00% in the control group,and the difference was statistically significant(P<0.05).Conclusions Laparoscopy in the emergency treatment of gastric perforation.can reduce the intraoperative blood loss,shorten the operation time and recovery time,and reduce the incidence of complications.Moreover,it is more effective than laparotomy.
