中药汤剂联合四联疗法治疗对消化性溃疡伴抑郁患者中医证候积分、心理状态及生活质量的影响

Effect of Chinese medicine decoction combined with quadruple therapy on TCM syndromes scores,psychological status and quality of life in patients with peptic ulcer complicated with depression

目的:探讨中药汤剂联合四联疗法治疗对消化性溃疡伴抑郁患者中医证候积分、心理状态及生活质量的影响。方法:选择我院2016年11月至2017年10月收治的消化性溃疡伴抑郁患者125例,按照随机数字表法分为对照组(n=62)和观察组(n=63),对照组给予四联疗法治疗,观察组在对照组治疗基础上给予中药汤剂治疗,比较两组疗效,疼痛消失时间、溃疡愈合时间及住院时间,治疗前后中医证候积分、心理状态及生活质量评分,不良反应。结果:观察组有效率96.83%,显著高于对照组的83.87%(χ^2=6.043,P<0.05);观察组疼痛消失时间、溃疡愈合时间及住院时间均较对照组的短(t=-4.262,-3.706,-3.511;P<0.05);治疗后,观察组中医证候积分低于对照组(t=-3.998,P<0.05),生活质量评分高于对照组(t=3.745,P<0.05);治疗后,观察组SAS和SDS评分均显著低于对照组(t=-4.408,-4.645;P<0.05);观察组不良反应发生率(6.35%)与对照组(9.68%)比较无明显差异(P>0.05)。结论:中药汤剂联合四联疗法治疗可降低消化性溃疡伴抑郁患者中医证候积分,调节其负性心理,提高生活质量。

Objective:To explore the effects of Chinese medicine decoction combined with quadruple therapy on TCM syndromes scores,psychological status and quality of life of patients with peptic ulcer and depression.Methods:A total of 125 cases of patients with peptic ulcer and depression who were admitted to our hospital from November 2016 to October 2017 were selected and divided into control group(n=62)and observation group(n=63)according to the random number table method.Control group was given quadruple therapy,and observation group was treated with Chinese medicine decoction on the basis of control group.The efficacy,the disappearance time of pain,the ulcer healing time and hospital stay,and the TCM syndromes scores,psychological status and quality of life score before and after treatment,and adverse reactions were compared between two groups.Results:The effective rate in observation group was significantly higher than that in control group(96.83% vs 83.87%)(χ^2=6.043,P<0.05).The pain disappearance time,ulcer healing time and hospital stay time in observation group were shorter than those in control group(t=-4.262,-3.706,-3.511;P<0.05).After treatment,the TCM syndrome score in observation group was lower than that in control group(t=-3.998,P<0.05)while the quality of life score was higher than that in control group(t=3.745,P<0.05).After treatment,the scores of SAS and SDS in observation group were significantly lower than those in control group(t=-4.408,-4.645;P<0.05).There was significant difference in the incidence rate of adverse reactions between observation group and control group(6.35% vs 9.68%)(P>0.05).Conclusion:Chinese medicine decoction combined with quadruple therapy can reduce the TCM syndromes scores in patients with peptic ulcer and depression,regulate the negative psychology,and improve the quality of life.
